Oversees the initial patient evaluation as it pertains to the therapy prescribed.<br/></p><p>2. Oversees the collection, analyzing, interpretation and evaluation of appropriate data relative to the patient&rsquo;s therapy. Oversees receiving and documentation of medication orders.<br/></p><p>3. Oversees the obtaining of histories and application of these along with other patient data to optimize nutritional support and/or drug therapy management.<br/></p><p>4. Oversees sets of therapeutic objectives and monitoring of patients for desired therapeutic responses.<br/></p><p>5. Responsible for 24-hour availability as required ensuring operational compliance.<br/></p><p>6.Oversees the evaluation of the significance of signs and symptoms relative to certain disease processes and makes recommendations for the modifications of nutritional support and drug therapy.<br/></p><p>7.Oversees the detection and evaluation of the clinical importance of adverse reactions and complications associated with home IV therapies.<br/></p><p>8.Oversees when the pharmacist provides counseling and answers questions from patients, family and health care professionals concerning nutritional support and drug therapy.<br/></p><p>9.Oversees the maintenance of thorough and appropriate documentation in the patient clinical chart of physician orders and communication with other health care professionals.<br/></p><p>10.Oversees the pharmacist when assisting the referring physician and institution in planning and implementing patient care to ensure a smooth transition from hospital to home.<br/></p><p>11.Oversees the participation of Team Conferences, Care Plans, Medication reconciliation, lab values within pharmacy policy.<br/></p><p>12.Maintains proficiency in the skills and theory of nutritional support and IV therapy by reviewing the literature and attending in-services, seminars and conferences.<br/></p><p>13.Supervises and coordinates activities concerned with infusion services: ordering, receiving, storing, inventorying, issuing, and shipping materials, supplies, tools, equipment, and parts, in the stockroom and warehouse.<br/></p><p>14.Oversees the preparation of mixing of sterile IV products such as TPN, chemotherapy, and antibiotics.<br/></p><p>15.Oversees the pharmacist and pharmacy technician(s). Participates in the training of staff and monitoring their performance and progress (relation to infusion therapy).<br/></p><p>16.Oversees the pharmacist&rsquo;s verification of accuracy of all work done by pharmacy technician(s).<br/></p><p>17.Participate in the Company&rsquo;s Performance Improvement program and participate in Company committees as requested by Administration/Director of Clinical Services.<br/></p><p>18.Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.<br/></p><p>19 Perform any additional duties as assigned to meet the needs of the department and organization.<br/></p>
