Position Overview:
Majestic Care LPNs render skilled technical nursing services to residents within an assigned unit/wing in support of medical care as directed by the nursing management team; supervises CNAs to ensure a high quality of care is provided to all residents in that unit. 
Key Responsibilities:<ul><li>
Initiate and lead team conferences in development of individualized nursing care plans.</li><li>

How You Will Make an Impact: The Medical Assistant provides essential support to both healthcare providers and patients; often the first point of contact for patients, contributing to a positive and reassuring patient experience. This role directly contributes to patient care by measuring vital signs, preparing patients for examinations, and assisting with minor medical procedures, which enhance the efficiency of healthcare delivery, creating a smoother and more patient-centric fertility journey.
What You Will Do: The Medical Assistant is responsible for the daily management of patients through administrative and clinical tasks to support the smooth operation of the Care Centers, Surgery Centers, and Laboratories. This position will coordinate with physicians, nurses, and other healthcare professionals to provide the essential services needed. The Medical Assistant reports directly to the Practice Administrator.<ul><li>
Prepare patients for examination, including TPR, weight, blood pressure, update health information, allergies, and current medication.</li><li>
Document patient vitals, assessment, encounter type, and provider into EMR.</li><li>
Communicate with physician and/or nurse practitioner, changes in patient condition, abnormal vitals, lab results, and abnormal patient response during testing/exam.</li><li>
Prepare minor procedure trays and assist with procedures (i.e., hysteroscopy, endometrial and/or punch biopsy, cyst aspiration, Pap smears, GC/chlamydia testing, HSG testing, etc.).</li><li>
Prep exam room respective to patient appointment and testing.</li><li>
Educate patient on procedure(s) and provide answers prior to procedure.</li><li>
Assist physician and/or nurse practitioner with patient examinations.</li><li>
Maintain patient workflow to meet physician/nurse practitioner scheduling requirements.</li><li>
Order and schedule lab tests as directed; review results and annotate appropriate disposition.</li><li>
Order prescriptions/refills for patients prior to procedure; notify patient upon completion.</li><li>
Disinfect room after each exam/appointment re-prep for next patient.</li><li>
Equipment maintenance and sterilization; identify any need for supplies/parts/repair and notify Practice Administrator.</li><li>
Order and maintain back-office supplies and reconcile monthly receipts/packing slips for submission to the business office.</li><li>
Scan signed consents, office appointment forms, laboratory, and pathology reports into EMR.</li><li>
Monitor medicine cabinet/refrigerator and order medications, as needed.</li><li>
Perform urine pregnancy test, document results in EMR, and communicate with physician, nurse practitioner, or rad technician prior to exam/procedure.</li><li>
Autoclave all used instruments, document size and type of instrument on package, change sterile/cleaning solution, and document in the soak-log.</li><li>
Clean stationary room equipment, underside of exam tables, and provide clean/linens/supplies for all patient rooms&rsquo; weekly minimum, or as needed.</li><li>
Maintain HIPAA and Confidentiality for all patient and clinic information, HIV confidentiality.</li><li>
Understand and assess potential domestic violence and child abuse issues and report concerns to Medical Director or Practice Administrator.</li><li>

Conduct visits per protocol for all patients with chronic conditions under the direction of the physician and addresses urgent and complex issues. Including Initiation of treatment and management of chronic illnesses including, but not limited to, HIV infection, Hepatitis C, substance abuse, asthma, bronchitis, bronchitis, diabetes, lipid disorders, and hypertension.</li><li>
Identify and delegate visits that can be handled by RN under the supervision of the physician and having notified the patient in advance</li><li>
Maintains a professional demeanor at all times, including demonstration of sound moral and ethical character, compassion, adherence to all agency standards for dress and decorum; reports for duty and signs in and out per agency protocol.</li><li>
Demonstrates proficiency of electronic medical record; including documentation, e-prescribing and coding.</li><li>
Complies with productivity expectations under the direction of the Medical Director.</li><li>
Delivers cost effective care, according to evidence based guidelines</li><li>
Generates appropriate referrals to ancillary and specialty services including medical, mental health and social based services.</li><li>
Maintains documentation and e-prescribing practices according to standards of the agency and regulatory agencies including but not limited to history and physical, social history and literacy level as well as care management.</li><li>
Completes and locks progress notes within 48 hours of client encounter.</li><li>
Ensure timely prescribing, orders, addressing labs, interpreting of test results, addressing telephone encounters, ordering and reviewing of specialty referrals and consultation reports.</li><li>
Ensure individualized care plan contains care recommendations, recommend frequency of visits, schedule for tests/screenings and red flags/plan of action</li><li>
Performs routine diagnostic and therapeutic procedures according to accepted practice standards.</li><li>
Assist with assigned PCP&rsquo;s panel in managing lab results, reviewing consult reports, and reviewing consultation reports as determined by the PCP.</li><li>
May administer medications according to the same procedure and protocols applied to the nursing staff</li><li>
Assists in communicating the management of transitions of members with complex care needs. Discusses and refers complicated patients outside of clinical expertise to appropriate physician.</li><li>
Provide health education and counseling during visits.</li><li>
Provides on-call telephone coverage on a rotating schedule.</li><li>
Assists with direction of care team with commitment to all team members practicing to the top of their license</li><li>
Follow all protocols for daily huddle</li><li>
Participates in CQI activities.</li><li>
Attend patient care related meetings as necessary, including but not limited to morning report, case conferences</li><li>
Manage coordination of care for individual clients by working with the available resources both internal and external to agency</li><li>

Certified by either the American Academy Nurse Practitioners or American Nurses Credentialing Center (ANCC)</li><li>
BLS Certified</li><li>
Achieves and maintains HIV specialist status</li><li>
Possess or obtain within first 90 days of employment a DATA Waiver for Buprenorphine Prescribing authority</li><li>
Accumulate a minimum of 15 HIV specific CME credits throughout the year and a minimum of 25 Credits per year to maintain certification requirements</li></ul>

Position Overview:
