Responsibilities:<ul><li>
Assist clients with daily living activities: bathing, dressing, grooming, toileting, and mobility</li><li>
Provide companionship and emotional support</li><li>
Perform light housekeeping, meal preparation, and medication reminders</li><li>
Follow the individualized care plan provided by the nurse</li><li>
Report any changes in the client&rsquo;s condition to the agency</li><li>
Maintain accurate and timely documentation</li></ul>
Requirements:<ul><li>
Valid New Jersey CHHA license (Required)</li><li>
High school diploma or equivalent</li><li>
Compassionate, reliable, and professional</li><li>
Excellent communication skills</li><li>
Ability to work independently</li><li>
Driver&rsquo;s license and reliable transportation preferred</li><li>
Must pass background check and health screening</li></ul>
