Pilsen Wellness Center is seeking a Mental Health Professional (MHP) to join our PATH (Projects for Assistance in Transition from Homelessness) team, funded by SAMHSA. PATH is a federally funded initiative, designed to connect individuals with SMI who are experiencing homelessness to mental health, substance use, housing, and support services. Services may include outreach, screening, treatment referrals, and housing assistance. The ideal candidate will work with individuals experiencing homelessness and living with Serious Mental Illness (SMI)&mdash;including schizophrenia, bipolar disorder, and major depression.

<div>The Behavioral Health Clinician is a Master&rsquo;s-level licensed or license-eligible therapist responsible for providing comprehensive mental health services, including assessment, diagnosis, treatment planning, and evidence-based therapeutic interventions. This role supports individuals experiencing a wide range of mental health conditions and may operate across multiple service lines, including outpatient therapy, crisis assessment and intervention, and specialized programming (e.g., Applied Behavior Analysis), depending on program assignment. RESPONSIBILITIES
