The Patient Care Technician &ldquo;PCT&rdquo; provides basic care to patients, performing all job functions in an ethical manner and ensuring patient safety, privacy, dignity, and confidentiality. This role also provides daily comfort measures and interactions with patients to create a positive care environment and experience. The PCT coordinates patient care with the Registered Nurse (RN), including effectively observing and communicating changes in patients&rsquo; conditions to the RN and responding appropriately to emergency situations. 
 
The Patient Care Technician interacts with and contributes to the professional development and image of all unit staff practicing in a Shared Governance model of nursing practice. Provides personal direct care for patients in collaboration with the RN. Observes and effectively communicate changes in patient&rsquo;s condition to the RN and responds appropriately to emergency situations.</li><li>
Utilizes a consistent, patient-focused care approach to meet patient needs and demonstrate initiatives to utilize a holistic approach to identify patient needs. Assists with the patient dining process by serving meals/feeding. Instructs patients in self-care and instrumental activities of daily living to provide skill practice including but not limited to bathing, dressing and daily grooming activities.</li><li>
Supports the plan of care by carrying out and recording specific nursing care procedures such as bowel program procedures, emptying catheter, measuring and recording intake and output, measuring with gain or loss, and providing emotional support and encouragement to all patients and families.</li><li>
Prepares and maintains a record of observations and care given. Documents information clearly and completely utilizing correct forms/systems in a timely manner according to Center policy.</li><li>
Screens and interviews patients.</li><li>
Measures and documents vital signs.</li><li>
Monitors and supports patient progress.</li><li>
Prepares patients for tests, therapy, and other activities.</li><li>
Demonstrates competency in time management, organization, prioritization and critical thinking by planning and organizing work assignments and prioritizing the specific needs of the patients.

The PCT will work on-site at DaVita Skilled Nursing Facility dialysis dens.
Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have significant experience within DaVita, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to GSD. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the Village.
What you can expect:
In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work in conjunction with dialysis RNs to provide dialysis care.
Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.
Complexity: DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners.
Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. You apply your &quot;core&quot; skills to monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. <div>The Telephonic Health Services Coordinator (HSC) serves as the primary point of contact for newly referred patients and plays a critical role in patient outreach, registration, enrollment, and ongoing telephonic engagement. This role supports patients with chronic conditions such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, hypertension, and related conditions.
As the first member of the clinical team that most patients interact with at AMC Health, the HSC is responsible for building trust, demonstrating empathy, clearly explaining Remote Patient Monitoring (RPM) services, and ensuring a smooth and compliant enrollment process. Following enrollment, the HSC collaborates closely with nurse care managers, nurse care associates, and other members of the clinical care team to support patients in meeting their health goals and adhering to care plans.
This position is essential to organizational growth, patient engagement, and fulfillment of client enrollment commitments.
Essential Job Functions

Patient Outreach, Enrollment &amp; Engagement
<ul><li>Conduct outbound outreach to newly enrolled prospective patients to explain AMC Health&rsquo;s Remote Patient Monitoring programs, services, and benefits.</li>
<li>Accurately describe program features, expectations, and value in a clear, compassionate, and patient-centered manner.</li>
<li>Engage patients telephonically to establish rapport, address questions, and encourage participation and adherence to monitoring programs.</li>
<li>Complete required telephonic interventions, including Welcome Calls and Post-Physician Visit Updates.</li>
</ul>
Care Coordination &amp; Clinical Support
<ul><li>Document patient information accurately and comprehensively in the electronic health record (EHR) in accordance with organizational policies and standards of practice.</li>
<li>Collaborate with nurse care managers and the broader care team to support patient care goals.</li>
<li>Track hospitalizations and notify nursing staff upon patient discharge, as required.</li>
<li>Coordinate patient assistance services as directed, including referrals for transportation, housekeeping, home modifications, food assistance, or other approved services.</li>
<li>Follow up with patients to confirm services have been initiated and are meeting expectations.</li>
</ul>
Communication &amp; Administrative Responsibilities
<ul><li>Respond to inbound patient, client, and provider inquiries and appropriately determine service needs.</li>
<li>Initiate outbound communications (phone, fax, email) with patients, clients, physician offices, and care providers.</li>
<li>Confirm and maintain accurate provider contact information, including phone and fax numbers.</li>
<li>Escalate unresolved patient issues or care-coordination concerns to leadership in a timely and appropriate manner.</li>
</ul>
Performance &amp; Compliance
<ul><li>Meet or exceed established performance metrics and service level agreements (SLAs), including but not limited to:
<ul><li>Call volume expectations and disengagement thresholdComply with all company policies, procedures, HIPAA requirements, and applicable federal and state regulations.</li>

<li>Bilingual (Spanish) preferred.</li>
</ul>
Education, Experience &amp; Technical Requirements
<ul><li>Preferred: Completion of a Medical Assisting program accredited by CAAHEP or ABHES.</li>
<li>Minimum experience requirements:
<ul><li>At least six (6) months of experience performing general duties as a Medical Assistant OR</li>
<li>Three (3) years of experience working in a clinic or comparable healthcare setting with direct patient contact.</li>
</ul></li><li>Knowledge of medical terminology and prior experience using electronic health record (EHR) systems.</li>
<li>Strong technical aptitude, including the ability to assist patients remotely with health devices and troubleshoot basic issues.</li>
<li>Ability to type a minimum of 50&ndash;60 words per minute while speaking with patients by phone.</li>
<li>Must be able to work schedules aligned to client-specific time zone requirements, as dictated by business needs.</li>
</ul>
Licensure and Certification Requirements
<ul><li>Active CNA certification or equivalent certification (e.g., Pharmacy Technician), in good standing.</li>
</ul>
Physical Demands &amp; Work Environment
<ul><li>Ability to work effectively in a fully remote environment.</li>
<li>Must maintain a dedicated home office that is private, quiet, secure, and free from distractions.</li>
<li>Reliable internet access that meets company minimum technical requirements.</li>
<li>This role is primarily sedentary and requires extended periods of computer and telephone use.</li>

 Part-Time Medical Assistant

 Perform a high volume of phlebotomy

 Maintain supplies for the blood drawing stations

 Ensure lab specimens are sent to proper labs
