The ACP &quot;care team&quot; model pairs a highly clinically experienced provider (ACP) with a PCP to collaborate closely by sharing the care and continuity management of the patient panel. The PCP's task queue provides the focus of this collaborative platform whereby the Advanced Care Partner devotes their work to keeping all categories within SLAs, provides real time responses to patient messages, follows up with chronic care monitoring and management, and intimately understands the patient panel &quot;as their own&quot;, such that complex patients are known well, medical issues are managed efficiently, highest quality conversations engage patients' trust, and continuity of care between the patient and two providers is seamless and exemplary. Further, the ACP care-team model promotes career-sustainability, mitigates the potential for decision fatigue, prevents practice isolation, and addresses burn-out risk factors. The program prioritizes well-being and belonging for both patients and PCPs.</p>

Advanced Care Partner Role:</b></p>
<p>
The Advanced Care Partner is a full-time, remote clinician who works business hours live online with a team of PCP peers, a clinical supervisor, and the Clinical Case Management team of Care Navigators and nurses, as needed, for highly complex cases. The ACP will partner with approximately 5 PCPs and their patient panels, owning these shared panels with longitudinal accountability over time.</p>

The ACP is responsible for maintaining task category SLAs, including but not limited to:</p>

Job Summary</b></div><div>
You will provide culturally competent, and strength-based peer support to youth, young adults, and families experiencing a behavioral or mental health crisis. Working alongside a Mental Health Care Professional, this position will utilize de-escalation techniques to stabilize the situation; assist in developing a Safety and Action Plan to address priority needs; and, where appropriate, transport individuals to care facilities or alternative placement in the community. Services are provided in client homes or safe community locations (hospital, overnight shelter, transitional housing). You will collaborate with co-workers, supervisors, outside professionals, and others involved in delivering the individual or family&rsquo;s care to meet safety plan needs and action plan goals. As part of a connected, 24/7 regional crisis program, you support care coordination by passing down outreach details and outcomes to our day teams.</div><div><br>
This position is with the Y Social Impact Center, the social services division of the YMCA of Greater Seattle (YGS), serving around 5,000 youth, young adults, and families every year in communities across King County, Pierce County, and various counties across WA. Through our programs we aim to strengthen young people&rsquo;s ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; establishing stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center&rsquo;s programs include homelessness prevention, housing, behavioral health, crisis response, foster care licensing, violence prevention, and employment. Since many of the young people we serve have been involved in the foster care, criminal justice, behavioral health, and/or homelessness systems, we truly value relatable lived experience of our staff and those applying for our positions as an asset.</div><div></div><div><b><br>
Position Type/Expected Hours of Work</b><br>
This is a full-time, overnight position in which staff are dispatched from their homes to various locations in the community. Dispatches arrive through a phone app, so staff need to be able to hear notifications, wake up, and respond in a timely manner with their shift partner. All crisis work is to be done in-person unless the individual/family explicitly requests a telehealth response.</div><div><br>
After Hours teams are scheduled to provide on-call support every night (weekdays and weekends), including holidays. <b>The hours of work are typically from 6/7:00 p.m. to 8:00 a.m. the following morning. </b>Flexibility is required with some weekend daytime work required as job duties demand.</div><div><br>
