Position Summary
The primary purpose of the Resident Care Manager (RN) is to develop, evaluate, implement health care plans for individual patients, assist with management of individual medical plans, while providing the highest level of patient care. The Resident Care Manager (RN) also communicates treatment plans between families, doctors, providers and residents/patients.

Essential Functions<ul><li>
Evaluates updates resident/patient health care plans to achieve person-centered care.</li><li>
Conducts assessments for new residents/patients.</li><li>
Actively participates in clinical meetings.</li><li>
Collaborates with other members of the health care team, as needed.</li><li>
Utilizes the electronic medical record to establish &amp; validate parameters are established as indicated.</li><li>
Establishes, monitors, and documents person-centered care, as indicated.</li><li>
Uses SBAR to communicate patient&rsquo;s change of condition with physician.</li><li>
Reviews test results from medical exams.</li><li>
Implements physician orders and follow up with treatment plans.</li><li>
Communicates with families, health providers and patients. Receives consent, as required.</li><li>
Documents in the medical record to clearly reflect patient care &amp; current medical condition.</li><li>
Ensures completeness of medical records.</li><li>
Provides emotional support to families and patients.</li><li>
Provides education about health care plans to patients and families.</li><li>
Collaborates with other clinical staff to successfully implement patient plans of care &amp; provides excellent customer service.</li><li>
Ensures punctuality and regular attendance for assigned shifts.</li></ul>
Other Functions<ul><li>
Obtains accurate information from physicians, residents/patients, and payor source(s) regarding the expected discharge plan and communicate this information to the interdisciplinary team at the facility.</li><li>
Communicates information to care team and coordinate patient's smooth transition to the next level of care.</li><li>
Coordinates referrals from hospitals' social service and discharge planning departments, physicians, case managers, insurance companies and other referral sources.</li><li>
Conducts job responsibilities in according with the standards set out in the company&rsquo;s Code of Conduct, its policies and procedures, applicable federal and state laws, and applicable professional standards.</li><li>
Performs other duties as assigned.</li></ul>
Knowledge/Skills/Abilities<ul><li>
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.</li><li>
Strong communication, excellent customer service, teamwork and organizational skills.</li><li>
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.</li></ul>
Requirements:

Education
Degree in Nursing from an accredited nursing school. RN required.
Licenses/Certification
Current RN license.
Valid CPR certificate.
Experience
Six months experience in a long-term care environment preferred.
Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.</div>

This position is on our Medical/Oncology floor (5 South) located at our Regional Medical Center off I-184 and Curtis Rd. This position cares for a wide variety of patients, including oncology/cancer patients, medical/surgical, dialysis, psych, dementia, isolation/infectious disease (including negative isolation rooms), remote telemetry, palliative, and hospice care. Schedule: 3 shifts per week from 7pm-7:30am. Must be flexible to work a varied schedule that consists of weeknights, weekends, and holidays as needed.
 
WHAT YOU WILL DO:

<li>You will support clinical staff at the unit level by performing clerical, receptionist and patient service coordination.</li>
<li>You will perform various nursing care services and related non-professional services necessary in caring for the personal needs and comfort of patients, as those of the Certified Nursing Assistant (CNA) or Unlicensed Assistive Personnel (UAP) under the direction and supervision of the licensed nurse.</li>

MINIMUM QUALIFICATIONS:
