Sentara Heart Hospital located on the campus of Sentara Norfolk General Hospital in Norfolk, Virginia is hiring a Transplant Coordinator Registered Nurse for the Advanced Heart Program!
The Transplant Coordinator plays a critical role in facilitating the complex logistics of organ transplantation while prioritizing patient safety and well-being while ensuring a smooth and efficient journey for both the donor and recipient.
The Transplant Coordinator coordinates the clinical management of the transplant/VAD patient including the transplant/VAD evaluation, the transplant/VAD process and follow-up care. Provides clinical leadership to care team members, allowing staff to plan, direct, manage outcomes and evaluate care.
Advanced Heart Failure Unit:
<ul><li>24 bed Specialty Unit that provides high acuity cardiac care to patients with severe heart failure, pre/post heart transplant, the Total Artificial Heart, and pulmonary hypertension.</li><li>
Work with high tech equipment such as mechanical circulatory support including ventricular assist devices (LVAD, BiVAD) and telemetry monitors.</li><li>
Environment relies on 24 hours consistent communication and intricate interdisciplinary teamwork skills.</li><li>
Nurses handle 3-4 patients on this unit based on acuity level.</li></ul>
Education<ul><li>
Bachelor of Science Nursing- BSN (required)</li></ul>
Certification/Licensure<ul><li>
Virginia RN License Virginia or Compact/Multi-State Eligible.</li><li>
All Direct Care RN's required to have BLS within 90 days of hire.</li><li>
Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire.</li></ul>
Experience<ul><li>
 Two (2) years of related nursing or transplant experience required.</li></ul>

Sentara Norfolk General Hospital is hiring a Supervisor, Violence Intervention Program to join their team.
The position is full-time 40 hours per week/day shift.
Requirements:
Bachelor's degree in Public Health/Social Work/LCSW or related fields required. Master's preferred.
3 years of community health experience
3 years of Supervisory experience required
Prior experience with grant management
The Hospital based violence intervention program coordinator is primarily responsible for programmatic oversight and management for a multi-disciplinary hospital-based violence intervention program dedicated to reducing re-injury and mortality rates among youth and adults. The program coordinator will develop a program using trauma informed practices and incorporating the social determinants of health.
