CLINICAL PRACTICE

7. Initiates referrals to specialty services and fosters continuity of care.

8. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery.

9. Maintains patient/employee confidentiality in the management of information.

Qualifications
At least two years of nursing experience in area of specialty practice applied for. Nurse Practitioner experience of at least one year preferred

Licensure: New York State License with current registration as a registered nurse.

State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows:

a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination.

b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course).

Education:

Position Summary

We are seeking an energetic and empathetic Licensed Midwife- (Clinical Coordinator III) to join the Department of Obstetrics and Gynecology at Columbia University Irving Medical Center. This full-scope role includes the provision of outpatient care at 1111 Westchester Avenue located in White Plains, NY and inpatient care at The New York Presbyterian Westchester Hospital. Alongside attending physician practice, the midwife will collaborate to deliver comprehensive obstetric and gynecological care. Ensuring the highest quality of compassionate patient service, the Licensed Midwife will report administratively and clinically to the General Obstetrics and Gynecology Division Administrator and Collaborating Physician.

Responsibilities

Patient Care Management and Documentation
<ul>
<li>Participates as a member of the healthcare team and interdisciplinary committees to improve patient care and meet patient needs.</li>
<li>Obtains and documents subjective history (interval or comprehensive) as appropriate to chief complaint or reason for visit.</li>
<li>Conducts relevant physical exams, documenting pertinent positive and negative findings.</li>
<li>Identifies a diagnostic impression and/or formulates a differential based on the subjective and objective data obtained.</li>
<li>Writes prescription(s) according to New York State and federal law.</li>
<li>Formulates a goal-directed plan that includes health maintenance, anticipatory guidance, and appropriate medical and nursing interventions according to established protocols/practice agreement.</li>
<li>Assesses and documents patient response to treatment and interventions.</li>

We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible to perform their job practicing good risk management, quality assurance, and delivering excellent internal and external customer service. The Nurse Practitioner assists with leading the care team and provides guidance to all team members. The Nurse Practitioner is expected to help lead the care team with a commitment to ensuring each team works to the top of his or her license consistently adheres with all protocols. Essential Job Functions
The following duties are mandatory requirements of the job:<ul><li>
Provide excellent customer service at all times, apologizing for any throughput delays, engaging patients with a warm and friendly greeting.</li><li>
Provide direct patient care, including prevention, screening, diagnosis, and treatment of illness, within their licenses scope of practice and specialty training</li><li>
Provides Primary Care, routine and acute care, for scheduled, same day and walk in appointments for face to face encounters as well as telehealth encounters.</li><li>
Conduct visits per protocol for all patients with chronic conditions under the direction of the physician and addresses urgent and complex issues. Position Summary

The Department of Neurological Surgery is seeking to recruit an experienced outpatient Nurse Practitioner to provide clinical care and clinical research in collaboration with the attending physician. The Nurse Practitioner will be responsible for evaluating and managing patients with neurological conditions that may require neurosurgery.

Responsibilities

Responsibilities include but are not limited to:

<li>Perform and document a complete medical history and physical examination to formulate diagnoses and treatment plan.</li>
<li>Order and review appropriate laboratory tests and imaging studies.</li>
