Founded by the New York City Department for the Aging (DFTA) and the New York State Office for the Aging (SOFA), LSNC Aging Services provide comprehensive support to over 800 active members, fostering health, engagement, and social connection. Older adults (ages 60+) benefit from daily nutritious hot lunches, wellness programs promoting physical and mental health, educational and arts programming to encourage lifelong learning and social services. Naturally Occurring Retirement Community (NORC) or LSNC NORC specifically serves residents aged 60+ living in the Amsterdam Houses and Amsterdam Addition&mdash;a NYC Housing Authority (NYCHA) development. <b>Essential Duties & Responsibilities:</b><br>
The essential duties of the Case Manager include but are not limited to the following activities:

Position Summary:</b></p><p>
This position is responsible for obtaining and logging case management (CM) assessments/reassessments and HIV Risk assessments for a targeted cohort of Amida Care&rsquo;s HIV-Negative, Transgender and Non-binary (TGNB) membership. This position will also be responsible for conducting essential case management responsibilities for this portion of the membership, including TGNB health navigation, HIV prevention health education, and referrals to community partners to address essential social determinants of health. Additionally, this position will work with external case management agencies and care teams to collaborate on case management needs and to obtain and record necessary documentation. and progress notes received from external case management agencies, by educating sites and collecting documentation.</p><p><b>
Responsibilities:</b></p><ul><li>
Conduct telephonic reassessments and HIV risk assessments every 180 days for assigned caseload.</li><li>
Refer members to the appropriate case management agency after assessment completion, if requested by member.</li><li>
Perform monthly generation and on-going review of case management site rosters and identify members who are actively receiving case management.</li><li>
Obtain comprehensive psychosocial case management assessments/progress notes for those active members assigned to a case management agency.</li><li>
Document elements of the assessment, including the needs identified and member&rsquo;s input.</li><li>
Review and address Gaps in Care with members and providers as applicable.</li><li>

A Brief Overview</b></div><div></div><div><br>
The Case Manager (&quot;CM&quot;) provides vital administrative and operational support to panelists who handle arbitrations and mediations while ensuring smooth case management processes and experiences. The CM focuses on mastering the fundamentals of case management while maintaining excellent client relationships and contributing to the overall success of the alternative dispute resolution (&quot;ADR&quot;) process.</div><br>
Responsibilities:

What you will do</b></div><ul><li>
Manages case files, maintains accurate records and ensures that all case-related documents are up to date.</li><li>
Prepares and distributes panelist lists in response to arbitration filings or client requests, in partnership with local management, the Arbitration Practice Team, and ADRC, ADRS, and SCM colleagues.</li><li>
Schedules and coordinates hearings, conference calls, and other case-related activities to meet client and panelist needs.</li><li>
Provides exceptional client service by responding promptly to general inquiries and website information requests.</li><li>
Supports their assigned panelists with basic administrative tasks related to their ADR practices; ensures timely follow-up on case-related actions.</li><li>
Monitors case timeliness and deadlines; ensures all milestones are met.</li><li>
Communicates effectively with clients, panelists, and team members to resolve basic questions or issues that arise during the case lifecycle.</li><li>
Engages in learning opportunities to expand knowledge in the company&rsquo;s processes and tools to become proficient in case management systems and workflows.</li><li>
Builds foundational knowledge of ADR practices and procedures through training and hands-on experience.</li><li>
Other duties as assigned within similar scope.</li></ul>
Qualifications:
<ul><li>Bachelor's Degree in Business, Operations, Management, or related field. Preferred</li><li>
2-4 years of working in case management. Required</li><li>
2-4 years of working in legal and client service role. Required</li><li>
1-3 years For internal candidates, proven ability as a Case Coordinator, or similar position. Required</li><li>
Proficiency in all ADR processes and procedures including mediation, arbitration and court reference matters. (Required proficiency)</li><li>
Knowledge of Panelists&rsquo; proficiency, practice areas and preferences in the local Resolution Center (RC). (Preferred proficiency)</li><li>
Computer literate and proficient in all software programs required for the position. (Required proficiency)</li><li>
Written and verbal communication skills. (Required proficiency)</li><li>
Emotional intelligence and adaptability under pressure. (Required proficiency)</li><li>
Ability to organize, prioritize, and manage multiple responsibilities and tasks in a quick-paced environment. (Required proficiency)</li></ul></div>
