<b>POSITION SUMMARY</b><br>
We are seeking Behavioral Health Therapists for our clinic locations in Altgeld Gardens, Hegewisch, and Hammond. The Behavioral Health Therapist is a member of the interdisciplinary team and contributes to the comprehensive health care services offered by TCA Health. The Behavioral Health Therapist provides services to individuals, families, and groups to assist with improvement of social functioning and maximizing strengths. The Therapist interacts and interfaces with the community in diverse roles of advocacy and overall planning for service utilization. This position may be a regular full-time or a regular part-time employee. This position will be on-site.<br>

<b>Behavioral Health Therapist Responsibilities</b>
<ul><li>Perform bio-psychosocial assessments and diagnosis of behavioral health clients.</li>
<li>Provide effective treatment planning to assist clients in successfully achieving goals.</li>
<li>Evaluate crisis situations and apply appropriate interventions.</li>
<li>Assist in the detection of &ldquo;at risk&rdquo; patients and the development of plans to prevent further psychological or physical deterioration.</li>
<li>Initiate treatment interventions in individual, family, and/or group counseling settings.</li>
<li>Effectively and clearly document client interactions and referrals in a timely manner as required.</li>

The Institute for Nonviolence Chicago is currently seeking a Behavioral Health Specialist to join the Behavioral Health and Wellness team and assist with the development and implementation of Behavioral Health and Wellness programming and services. This position will be responsible for daily service delivery of staff wellness programming and participant behavioral health and wellness services for &ldquo;in-risk&rdquo; individuals, victims, and their families. The ultimate goal is to provide wellness programming to staff to improve employee health and program capacity, as well as provide behavioral health services to individuals who are actively involved in street violence, or recently impacted/exposed to community violence. <p></p><p>OUTPATIENT ORTHOPEDIC PHYSICAL THERAPIST POSITION: A full‑time outpatient Physical Therapy position at UI Health specializing in orthopedic and musculoskeletal care.<br>

The Physical Therapist is responsible for the provision of physical therapy services within her or his individual scope of clinical competence including: examination, evaluation, diagnosis, prognosis, development and implementation of a plan of care that includes direct intervention, communication, collaboration and patient education and the evaluation of outcomes.<br>

<b>Duties &amp; Responsibilities:</b></p><ul><li>
Performs the duties of a professional Physical Therapist for an assigned caseload with emphasis on providing comprehensive care.</li><li>
The PT is responsible for upholding professional standards and promoting quality in practice.</li><li>
Direct Patient Care time (actual time with patients, may vary from scheduled) - includes all direct interventions and patient education that is integrated as a part of treatment session</li><li>
Indirect Patient Care (IPC) - time spent in chart review, documentation (direct patient care documentation<br>
as well as any other documentation related to patient care), scheduling, attending team or family meetings<br>
and rounds (documented as clinic time), ordering equipment, assisting other staff</li><li>
Responsible for administrative tasks related to the patient care services they provide, related to department operations, or required for regulatory and organizational compliance. This includes time sheets, case presentations, staff meetings, committee meetings, giving inservices.</li><li>
Responsible for professional development and skills for self as well as sharing expertise and participation in development of the role of physical therapy throughout the health system and within the community.</li><li>
Responsible for clinical teaching for students and student clinicians.</li><li>
