 The Care Coordinator (CC) &ndash; I-SNP is a clinical support and care coordination role responsible for&nbsp;enabling&nbsp;front-line clinicians&nbsp;to deliver&nbsp;high-quality psychiatric and medical care to Medicare Advantage Institutional Special Needs Plan (I-SNP) members&nbsp;residing&nbsp;in long-term care&nbsp;skilled nursing facility&nbsp;settings.  The majority of&nbsp;the role is focused on front and&nbsp;back-office&nbsp;support, including managing inbound communications, coordinating care, and supporting clinical workflows.  JOB SUMMARY: The Nurse Care Coordinator delivers and oversees care management services for medically and/or socioeconomically complex patients in accordance with patient-defined goals, multi-disciplinary plan of care, and established policies and procedures. Drawing on best practices in motivational interviewing and care management, the Nurse Care Coordinator collaborates with clients and multi-disciplinary teams to develop and implement flexible, patient-centered, and cost-effective strategies that support clients to achieve health-related goals. Performs specific care activities such as specimen collection phlebotomy intravenous (IV) catheter insertion electrocardiography (EKG) and documentation in the health record.Incumbents in this position may perform all or a combination of the duties described depending upon the assigned work area and the specific needs of the patients and the department.
Primary Duties and Responsibilities 
<ul><li>
Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality and safety standards and SPIRIT Values. Complies with governmental regulations and accreditation standards.</li><li>
Assists patients with activities of daily living (ADLs) and personal hygiene including bathing mouth care dressing skin care positioning feeding ambulation toileting and range of motion activities. The Clinical Research Coordinator administratively coordinates clinical protocol implementation, typically for a single study. Will ensure efficient logistical implementation of the study activities and provide administrative support for research-related regulatory issues. 
Specific Duties &amp; Responsibilities<ul><li>
Coordinate all activities of a clinical research study to ensure adherence to protocol and validity of findings.
</li><li>Participate in clinical study start-up meetings.
</li><li>Assist research nurses, study coordinators, principal investigators, and other study personnel in executing protocol-related activities.</li><li>
Explain the study background and rationale for the research to potential and current participants.</li><li>
Contribute to the development of recruitment strategies for participants for assigned study.
