Overview:</b></p>
<p>
The Housing Works Inc. Alternative to Incarceration (ATI) Program is a collaboration between Housing Works and the Mayor&rsquo;s Office of Criminal Justice (MOCJ) to provide individuals 18 years of age and older with open legal cases with comprehensive, holistic behavioral health services in lieu of jail or prison. Individuals with open cases in Brooklyn or Manhattan criminal or supreme courts are eligible for the following court-mandated services: case management, substance use and mental health services, individual counseling, group level interventions and peer support.</p>
<p>
Reporting to the Clinical Coordinator, the Counselor will be primarily responsible for providing individual counseling for clients with a history or active substance use or behavioral health needs, facilitating evidence-based group level interventions, supporting clients non-compliant with the court mandates and promoting engagement and retention in program services. The ideal candidate will have an active NYS CASAC license or previous experience providing substance misuse or behavioral health counseling and experience providing services for individuals within the court-mandated treatment sector.</p><br>

<p><b>
Responsibilities:</b></p>
<p>
Primary:</p>
<ul>
<li>Assist with conducting LCSMI risk-needs assessments with newly referred clients as needed.</li>
<li>Provides individual counseling in accordance with clients&rsquo; court mandates with clients assigned to caseload.</li>
<li>Facilitates three to four evidence-based group counseling sessions weekly.</li>
<li>Participate in interdisciplinary case conferences and care coordination activities.</li>
<li>Work closely with clients to promote positive engagement in services and to maintain compliance with court mandates.</li>
<li>Engage in crisis intervention as needed.</li>
<li>Participate and support clients through discharge planning and connection to after care services prior to program completion.</li>
<li>Thoroughly and frequently document client engagement and services provided within the electronic record system.</li>
<li>Actively participate in program, department and agency meetings.</li>
<li>Promotes Housing Works&rsquo; core values in day to day activities.</li>
<li>Performs other tasks as assigned by supervisor</li>
</ul>

<li>Provide culturally competent services in accordance to Housing Works policies and practices.</li>

Requirements:</b></p>
<ul>
<li>Associate&rsquo;s Degree required in social services, psychology, social work or other related field.</li>
<li>Active NYS CASAC license preferred.</li>
</ul>

<li>Minimum 1 year experience providing individual or group level counseling.</li>
<li>Experience working with individuals involved or impacted by the criminal legal system.</li>

<br>The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. PROGRAM DESCRIPTION
<br>The ACT programs provide an integrated set of evidenced based treatment, rehabilitation, case management and support services to individuals residing in NYC, in Shelter programs, and/or living in supportive housing programs or their own residences in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.
<br><br>
<b>POSITION OVERVIEW:</b><br>
This multi- faceted position works directly with adults living with severe and persistent mental illness (SPMI) and substance use disorders (SUD) living in NYC. In addition to providing direct care, this position provides administrative oversight and support to the treatment team to ensure the delivery of services is in accordance with regulatory and payer requirements. Clinical Coordinators use a recovery oriented approach to link recipients to the services they need to obtain housing and to achieve wellness self-management and increasing independence. Working in a team setting and primarily in the community, Clinical Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate clients and coordinate other aspects of clients’ health and community services. <b>KEY ESSENTIAL FUNCTIONS:</b><br>
<ul>
<li>Assume administrative tasks such as chart auditing, collaborating with outside partners, facilitating team meetings and being the point person in crisis situations when Program and Team Director are not available.</li>

<li>Assist with on-boarding/orientation of new staff</li>

<li>Attend mandatory ACT trainings and Jewish Board MKSEI 13 Module course training.</li>
<li>Collaborative development of an Individualized Care Plan (ICP) with the client, the client’s family and/or caregivers in addition to other service providers, and review and revise ICP per regulatory standards or when there are significant changes in functioning or clinical status.</li>
<li>Provide care management services</li>
<li>Track all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided.</li>
<li>Assure that client has access to, engages in and retains needed services as defined in the ICP.</li>
<li>Provide outreach services to clients to increase engagement in services identified on individualized care plan</li>
<li>Complete all required documentation</li>
<li>Provide after-hours emergency coverage via cell phone on rotating basis.</li>

<b>EDUCATIONAL / TRAINING REQUIRED:</b><br>

<li>Obtained a Master’s level degree in human services field with appropriate NYS licensure (e.g. MSW, LMSW, LMHC, LCSW, LMFT, Creative Arts Therapist LCAT, licensed Occupational therapist, Certified Psychiatric Rehabilitation Counselor, Rehabilitation Counselor, licensed psychologist or hold a Master’s Degree as a Therapeutic Recreation Specialist or Psychologist or Pastoral Counselor or Master’s degree in a related field.</li>

<p>Position Summary</p>

The Case Manager reports to the Associate Director of Case Management and serves as a key member of Disability Services Case Management team. Under general supervision, the Case Manager will provide case management services for students with a range of disabilities enrolled in programs on the Morningside campus. This position is integral to furthering accessibility and inclusion for students with disabilities at Columbia and will work closely with faculty and University partners to remove barriers to learning, co-curricular programs and resources.</p>

Case Management (75%)</b></p>

The HLPS Case Manager (a &ldquo;CM&rdquo;) is an essential team member that develops and executes housing navigation and placement services including follow-up post placement for clients referred by NYC Health + Hospitals (H+H). The CM meets with the client in person to develop a housing plan and complete the housing packet and works to support the client to find and move into permanent housing.
