 <p>
  Health Home Plus (HH+) is an intensive Health Home Care Management (HHCM) service established for defined populations with Serious Mental Illness (SMI) who are enrolled in a Health Home (HH) serving adults.
 </p>
 <p>

  </strong>
 </p>

  <strong>
   What The Project Coordinator Does:
  </strong>
 </p>
 <p>
  The person filling this position is responsible for: aiding qualified individuals who require social service assistance. This includes interviewing and evaluating clients, formulating service plans and goals, in order to aid clients in meeting their Mental Health and Medical needs. This should be done through the use of appropriate community resources/referrals. Field work is mandatory, each client must be seen a minimum of 2 times per month or as needed, specific to client need.&nbsp; Project Coordinators are expected to provide intensive case management services to select populations&nbsp;
  <em>
   (as defined by the New York State Department of Health)&nbsp;
  </em>
  who also have a Serious Mental Illness (SMI) and are enrolled in a Health Home (HH) serving adults. Case Management services are provided for a period of up to 1 year, possibly longer, dependent on client need.
 </p>

  <li>
   Carry and maintain a caseload of up to 20 clients, providing assessments, developing care plans, provide service referral/navigation, and crisis intervention, as needed
  </li>
  <li>
   Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information
  </li>
  <li>
   Conduct required field work. All clients must be seen, in-person, a minimum of two times per month. Additional visits may be required depending on client need
  </li>
  <li>
   They would also require two additional telephonic calls, for a total of 4 encounters per month
  </li>
  <li>
   Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency
  </li>
  <li>
   Recommend and implement strategies to persuade clients to participate more fully in this process
  </li>
  <li>
   Monitor clients&rsquo; progress toward their service plans goals via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via service plan outcomes and detailed progress notes (i.e. time of service, type of service, etc.)

   Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, entitlements, etc
  </li>

   Escort clients to appointments (educational, medical, social service, etc.)
  </li>

   Follow-up with clients for a period of time after successful completion of their primary goals to assure client stability
  </li>

   As necessary reach out and market the program to the community in order to recruit clients

   As needed prescreen clients over the telephone for eligibility and may schedule appointments for possible enrollment

   Assist clients in completing applications for benefits and entitlements and may process applications on clients&rsquo; behalf as needed

Adheres to intensified monthly core services and face-to-face engagement thresholds established for each sub-program (e.g., Health Home Plus (4 core services / 2 FTF), AOT (4 core services / 4 FTF), NYC Department of Health &amp; Mental Hygiene (DoHMH) Grant (2 core services / 2 FTF).</p><p>
5) Convenes case conferences (every six months at minimum), ensuring each patient's care team is up-to-date on patient's status and needs. Responds promptly to all care team member inquiries and ensure that all discussions are documented in the electronic medical record (EMR). Prioritizes case conferences for any patient exhibiting signs and symptoms of behavioral health crisis or relapse to use of substances or alcohol.</p><p>
6) Maintains expertise in approved techniques for deescalation, crisis management, and safety planning to effectively monitor and support patients in managing triggers, coping with stressors, and seeking crisis support. Proactively develops safety plans for all enrolled patients, and updates them at least annually.</p><p>
7) Monitors and responds to critical alerts (e.g., inpatient hospital and emergency department admission / discharge, arrest/incarceration, etc.) in accordance with all related policies for supporting transitions of care.</p><p>
8) Utilizes a directory of service providers to support connection of patients to medical, behavioral health, social support, wellness and family support services, in support of meeting care plan goals and interventions.</p><p>
9) Develops and maintains proficiency in the EMR. Proactively manages daily schedule to balance travel time, appointment duration, and ensure documentation of all work completed daily.</p><p>

In addition to emergency and transitional housing, URI delivers a robust continuum of Justice Initiatives&mdash;including economic empowerment, legal education and advocacy, prevention and youth programming, and abusive partner intervention&mdash;designed to break cycles of violence, instability, and poverty. URI also develops permanent affordable and supportive housing, advancing long-term pathways to safety, healing, and stability for families and communities.</p>

Innovative Urban Village, opening in Summer 2026, is Urban Resource Institute&rsquo;s first Supportive Housing program in partnership with Christian Cultural Center and is a comprehensive redevelopment project located in East New York, Brooklyn. Phase 1A includes 376 income-based rental residences, of which 94 units are dedicated to families who have experienced homelessness as a result of domestic violence and who qualify for Empire State Supportive Housing Initiative (ESSHI) services. The program provides permanent supportive housing and trauma-informed, culturally competent services designed to promote long-term housing stability and independent living. POSITION OVERVIEW
</b></p><p>The Director, Clinical Services provides strategic clinical leadership and oversight for all behavioral health and trauma informed services within the Innovative Urban Village supportive housing program.
