Minimum Qualifications:

Role Summary: We are seeking a compassionate and dedicated Case Manager to join our team. Primary Job Responsibilities/Duties:

Overview:

The Case Manager will conduct intake interviews, care plan development, care coordination, referral services, substance use counseling coordination, health education, and social services navigation to members with substance use and other chronic health conditions. Other interventions are Healthy Conversations, AOD Counseling and Seeking Safety education as eligibility to deliver them allows. The Case Manager will act as an intermediary between providers and members &ndash; serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher, and more.
This role will work with Ryan White and Harm Reduction prevention services for high-risk of HIV populations. This position is scheduled for 9AM to 5PM from Monday to Friday 


Responsibilities:

Primary:
<ul>
<li>Completes initial assessment as needed and reassessments on all assigned clients in a timely manner in-order to develop individualized treatment goals for each client.</li>
<li>Establishes treatment goals and objectives with clients as part of the interdisciplinary team.</li>
<li>Provides case management services including but not limited to weekly and monthly progress summaries, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site &amp; off-site) and discharge planning.</li>
<li>Facilitates harm reduction individual and group services as needed.</li>
<li>Attends weekly/bi-weekly supervision meetings as agreed upon by the Program Coordinator.</li>
<li>Ensure timely submission of clinical documentations to care organizations for uninterrupted services.</li>
<li>Coordinate transitional care from inpatient service to other settings as per care plan.</li>

 The mission of the Bureau of Tuberculosis Control (BTBC) is to prevent the spread of tuberculosis and to eliminate it as a public health problem in New York City. The goals to achieve this mission are twofold: 1) To identify all individuals with suspected or confirmed TB disease and ensure appropriate treatment; 2) ensure that individuals at high risk for progression from TB infection to TB disease are identified and treated for TB infection and do not develop TB disease.
 
 
 
 Position Summary
 
 
 
 Reporting to the Supervising Public Health Advisor and or the Regional Manager, and working with an interdisciplinary team, the Public Health Advisor (Case Manger) will be assigned cases to manage from the time they are reported to BTBC until the patients complete TB treatment or disposition. The Public Health Advisor will interview and educate patients, communicate with medical providers, care coordination, collect data, update the TB registry, conduct home visits, and participate in QA reviews.

 Minimum Qualifications

 A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social or biological sciences; or
 </li>

 A baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data; or
 </li>
 <li>
 A four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described in "2" above; or
 </li>
 <li>
 Education and/or experience equivalent to "1", "2" or "3" above. Undergraduate college credit can be substituted for experience on the basis of 30 semester credits from an accredited college for one year of full-time experience. However, all candidates must have a four-year high school diploma or its educational equivalent, and either twelve semester credits as described in "1" above or six months of experience as described in "2" above.

Essential Duties and Responsibilities: 
The essential duties of the Case Manager include but are not limited to the following activities:
 <ul>
<li>Provide case management and counseling services by conducting intake, psychosocial and housing assessments of clients to identify and obtain financial entitlements, housing and other concrete services (Health Insurance, Food Stamps, SSI etc.)</li>
<li>In collaboration with each client, prepares and monitors an Independent Living Plan, identifying long and short-term objectives to be achieved</li>
<li>Identifies and makes referrals for community based treatment and support services</li>
<li>Counsels clients through individualized and group sessions on issues such as wellness self-management, maintaining sobriety, complying with medication, pursing employment, independent living skills etc.</li>
<li>Collaborates with on-site psychiatric provider/nursing/primary care to ensure each client has an aftercare plan</li>
<li>Maintains accurate and up to date written client documentation as required and in accord with city, federal, state and local agency regulations and guidelines</li>
<li>Inputs client information and updates into the database system</li>
</ul>
Qualifications: 
<ul>
<li>Bachelor’s Degree in Social Work or related field of experience required</li>
<li>1-2 years of experience working with homeless mentally ill adults or other special needs population preferred</li>
<li>Excellent oral, writing and listening skills</li>
<li>Proficiency in computer required.</li>
<li>Bilingual or multilingual a plus</li>
