Position Overview
 
Under the supervision of the Family Preservation Clinical Coordinator, the Case Manager provides comprehensive family assessments and delivers in-home, strengths-based services to families enrolled in the Family Preservation Program. This role partners closely with families and a multidisciplinary team to develop and implement individualized service plans that promote child safety, family stability, and overall well-being.  Reporting to the Program Manager, the Case Manager delivers high-quality, client-centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15&ndash;20 participants, serving as their central point of contact.
 
 
 
 Key Responsibilities

 Conduct initial and quarterly psychosocial assessments.
 </li>

 Collaborate with participants on individualized service plans and housing stability goals.
 </li>
 <li>
 Build trusted, professional relationships with clients.
 </li>
 <li>
 Provide outreach, housing navigation, and ongoing support.
 </li>
 <li>
 Facilitate referrals to healthcare, mental health, substance use, and community services.
 </li>
 <li>
 Support clients in obtaining employment, benefits, and education opportunities.
 </li>
 <li>
 Accompany clients to key appointments as needed.
 </li>
 <li>
 Use a harm reduction, strength-based, and client-focused approach.
