This includes reviewing the patient chart, taking a history, completing a physical assessment and interpreting diagnostic and radiologic testing along with communicating those findings to the PCP.</li>
<li>Perform preoperative evaluations.</li>
<li>Proactive patient communication, education and coordination of care between visits and schedule patient appointments for telehealth or in-person visits when appropriate.</li>
<li>Provide post discharge coordination to reduce hospital readmission rates and emergency room utilization.</li>
