Job Description Essential Duties and Responsibilities:

<li>Maintain a caseload of approximately 20 high acuity Clients for intensive case management services (up to 40 if low acuity).</li>
<li>Develop relationships with Clients with a focus on facilitating independence and maintenance of improved physical and mental health.</li>
<li>Provide crisis assessment and intervention.</li>
<li>Facilitate life skills and coping skills groups for Clients, as indicated.</li>
<li>Contribute clinical insight during weekly team case conferencing.</li>

<li>Collaborate with LA County Department of Mental Health (DMH) and Department of Health Services (DHS) programs to enhance coordination within Clients’ treatment team.</li>

<li>Complete comprehensive biopsychosocial assessments and functional needs assessments to guide individualized treatment planning.</li>
<li>Complete assessment of mental health symptoms and diagnoses to guide mental health service linkage.</li>
<li>Complete quarterly collaborative goal plans with Clients with the purpose of targeting areas that will improve quality of life and health outcomes.</li>

<li>Ensure all Client documents within physical charts are complete, accurate, current, and filed as guided by agency, department, and funder expectations.</li>

<li>Coordinate appointments, transportation, and follow-up services for Clients to improve access to primary health care, mental health care, substance use services, and other local resources in order to build sustainable community connections and reduce the need for emergency health care services.</li>

 We are seeking an
 
 experienced Nurse Case Manager
 
 interested in working with injured workers and worker&rsquo;s compensation cases to cover
 
 Los Angeles
 
 and surrounding communities. This
 
 role is a home-based position with travel, up to 2 hours one-way (4 hours round-trip), to doctors&rsquo; offices, hospitals, and various other locations.
 
 
 
 This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.

 Experience in worker&rsquo;s compensation catastrophic case management is preferred. Experience may include:

 Critical care experience, catastrophic injury/illness case management, and/or disability management

 Clinical experience in acquired brain injury, spinal cord injury, multiple trauma (including amputation), and/or burns
 </li>
 <li>
 Recommended certifications (CCM, CRRN, CDMS, CRC). REQUIRED within two years of the date of hire
 </li>

 Ability to occasionally work outside of traditional business hours (evenings or weekends) if needed to meet the needs of the claim

Position Description:PACS is seeking a motivated and community-driven Case Manager to join our Intensive Services team.This role is part of our Full-Service Partnership (FSP) program, providing intensive, community-based mental health services to individuals with complex behavioral health needs. Clients may experience serious mental illness, housing instability, co-occurring conditions, and other high-acuity challenges.The Case Manager plays a critical role in delivering hands-on, field-based support that promotes stability, independence, and overall well-being.
