Position Summary

The incumbent will be responsible for providing Vascular Surgery services in outpatient and inpatient settings, including assessing, diagnosing, prescribing, treating, and educating patients. Work closely with Vascular Surgeons and other clinicians in a team approach to patient care.

Responsibilities
<ul>
<li>Performs history and physical examinations on patients. In consultation with the patient&rsquo;s attending physician, establishes a presumptive diagnosis and initiates the work up of the patient by ordering appropriate laboratory studies. Under supervision, is responsible for the management of the patient&rsquo;s medical issues following diagnosis.</li>
<li>Assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.</li>
<li>Assist with ordering appropriate laboratory diagnostic and perform other screening tests.</li>
<li>Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness.</li>
<li>Assists in hospital rounds. Makes complete chart entries on patient transactions and records progress notes.</li>
<li>May serve as 1st or 2nd assistant in assigned area. Scrubs, starts IV&rsquo;s, monitors equipment and alerts surgeon of changes in patient&rsquo;s condition, and may close skin after operation. Acts as liaison between surgeon and surgical staff throughout the operation.</li>
<li>Attends to the patient in the Recovery Room or Intensive Care Unit. May provide postoperative instructions to patients.</li>
<li>Attends daily rounds and provides post-operative care on floor by checking for signs of post-operative complications, changing dressings, and removing sutures.</li>
<li>Performs minor surgery under direct supervision. Performs clinical procedures under the direction and supervision of the responsible physician such as venipuncture, arteriopuncture and electrocardiogram. May administer intravenous medications, fluids, blood and blood components, and local infiltrative anesthetic. May administer injections I.M., subcutaneous and intradermal. May perform intradermal skin tests, nastrogastic intubation and insertion of urinary catheters. May cleanse and debride wounds.</li>
<li>Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.</li>
<li>Develop and implement treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.</li>
<li>Providing guidance to patient/family regarding medications and treatments.</li>
<li>Educates patients regarding health promotion/illness prevention.</li>
<li>Initiates appropriate therapy in medical emergencies until the arrival of the physician.</li>

<li>Collaborates with other caregivers to ensure continuity of care of patients, and to attain the greatest efficacy in the use of hospital resources in managing patients.</li>
<li>Provides recommendations to appropriate community resources to meet patient/family needs.</li>
<li>Communicates appropriate case management information to other professionals and community agencies.</li>

Education, Certification, Computer and Training Requirements:<ul><li>

Valid Drivers license and Auto Insurance.</li><li>
Knowledge of current nursing practices and protocols.</li><li>
Previous experience working directly with physicians required.</li><li>
Ability to communicate in English, both orally and in writing.</li><li>

Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
 
Reporting to the Assistant Program Director, Transitional Housing the Clinical Coordinator, Transitional Housing supervises Case Managers and ensures the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. The Clinical Coordinator is responsible for client records are kept in compliance with regulatory and agency standards.
 The Clinical Coordinator works primarily during business hours Monday-Friday, with off hours availability in case of emergency or for special projects/initiatives.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES: 
<ul>
<li>Supervise a team of Case Managers</li>
<li>Provide regular, structured supervision meetings for direct reports</li>
<li>Promote effective strategies for case management following Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing and trauma-informed care models of practice</li>
<li>Development and implement individual service plans; ensure documentation of client contact and progress is comprehensive and timely</li>
<li>Interview and assess potential clients</li>
<li>Provide clinical direction and leadership to the program, help troubleshoot, problem solve, and strategize solutions around client engagement and challenging behaviors</li>
<li>Promote a staff and resident culture that emphasizes housing placement as a measurement of success; ensure that clients are ready for housing placement</li>
<li>Oversee development and implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely</li>
<li>Assess and evaluate client functioning</li>
<li>Supervise and conduct case conferences</li>
<li>Coordinate delivery of care with multiple service providers, particularly outreach teams.</li>
<li>Work collaboratively with outreach teams to ensure individuals succeed in transitional housing and avoid return to homelessness</li>
<li>Provide support and guidance to front line staff in managing difficult situations and successfully de-escalating conflict</li>
<li>Perform other duties as assigned</li>
</ul>
MINIMUM QUALIFICATIONS: 
<ul>
<li>NYS Licensed Masters Social Worker (LMSW) required. Must obtain LMSW within first six months of employment</li>
<li>Minimum 3 years of experience with related populations; previous supervisory and administrative experience preferred</li>
<li>Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients</li>
<li>Experience with crisis intervention, including, risk assessments and incident management</li>
<li>Ability to work successfully with a wide range of internal and external stakeholders</li>
<li>Demonstrated success in working in a fast-paced environment with multiple priorities</li>
<li>Ability to delegate and motivate staff to achieve deliverables</li>
<li>Ability to communicate (verbally and written) with diverse populations and stakeholders</li>
<li>Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; familiarity with Access-based databases and the ability to learn new programs are required</li>
