Summary of the Position:</b></div><div>
Reporting to a Portfolio Manager, the Contract Manager is responsible for all aspects of the contracting processes. The Contract Manager will coordinate with internal and external stakeholders, monitor contractor performance and spending, and provide technical assistance to promote effective program implementation.</div><div></div><div><b><br>

Conduct telephonic reassessments and HIV risk assessments every 180 days for assigned caseload.</li><li>

<b>Minimum Qualifications:</b><br>

<li>Associate's degree or 60 college credits courses in Human Services field, or High School Diploma or GED with 2 years of relevant shelter experience.</li>
<li>The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.</li>
</ul>
<b>Case Manager Principal Responsibilities:</b><br>
1. Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
<br>2. Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
<br>3. Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
<br>4. Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.

The Managed Care Contracting Team within MSHP is responsible for the negotiation, implementation and contract compliance related to Facility Contracts for all Commercial, Medicare Advantage and Medicaid Managed Care Plans. The team serves as MSHP&rsquo;s subject matter experts on: Facility related Agreements and Amendments, financial performance for all contracts, and facility, ancillary related contract negotiations. The Managed Care Team is at the forefront of implementing Facility and Ancillary Contracts across the Mount Sinai Health System.<br>
MSHP seeks a Senior Contract Manager Analyst will play a critical role in testing, coding, modeling, and reporting on Hospital Billing (HB) and Professional Billing (PB) contracts for the MSHS Health System. This individual will ensure the accuracy of contract terms, reimbursement structures, and facilitate the flow of contract data into the revenue cycle system. A strong background in contract coding, logic, and the ability to interpret contract terms and reimbursement guidelines is essential. Revenue cycle experience is highly desirable.</p><b><br>
