Develop, implement and manage the referral process of youth and community members who experienced harm from the King County Prosecuting Attorney&rsquo;s Office (PAO) to KCYDI contracted community-based services;</li><li>
Contact referred youth and/or their families to engage them and refer them to community-based navigation services from contracted community-based organizations;</li><li>
Partner with the Black, Indigenous and People of Color (BIPOC) community-based organizations providing KCDYI navigation services to standardize the intake and assessment process;</li><li>
Work with partners to identify system barriers and strategies to drive continuous quality improvement that leads to better outcomes for youth/young adults (decreased involvement in the legal system; positive youth development; positive cultural identity; completion of secondary credentials, enrollment into post-secondary, persistence in post-secondary, employment);</li><li>
Work with County stakeholders, including, but not limited to, Department of Public Defense, Prosecuting Attorney&rsquo;s Office, King County Superior Court, Care First and other Department of Community and Human Services (DHCS) staff to ensure that youth have access to the services and programming they need to meet their goals and avoid further involvement in the criminal legal system;</li><li>

<div>The Prevention Division of Public Health &ndash; Seattle &amp; King County is seeking an <b>Educator Consultant I (Perinatal Hepatitis B Prevention)</b> to join our team. This a Career Service position that reports to the Adult Immunization Team Lead in the Immunization Branch and works collaboratively with staff from the Communicable Disease and Immunization (CD-Imms) Section&rsquo;s four branches &ndash; Immunization, Investigations, Outreach, Response and Education (IORE), Administration, and Analytics and Informatics (A&amp;I). The Educator Consultant focuses on education and outreach to internal and external partners on hepatitis B and prevention recommendations, including the Perinatal Hepatitis B Prevention Program (PHBPP).</div><div></div><div><br>
This position is responsible for a broad range of activities including developing and strengthening collaborative partnerships with health care facilities and providers, community-based organizations, and coalitions; enhancing hepatitis prevention efforts through education and outreach to communities at increased risk of hepatitis B; providing technical assistance and education to birthing hospitals, healthcare providers, and community partners to ensure infants complete the full hepatitis B vaccine series and post-vaccination serological testing; and supporting other work to promote the benefits of hepatitis B and other routinely recommended childhood immunizations. This position is part of Best Starts for Kids (Best Starts), a King County voter-approved initiative to support every baby born or child raised in King County to reach adulthood happy, healthy, safe, and thriving. Immunization team members conduct a variety of educational and outreach programs for children and adults. Background</b></div><div>
The Communicable Disease Epidemiology and Immunization (CD-Imms) section resides within the Prevention Division here in Public Health &ndash; Seattle &amp; King County. The CD-Imms section focuses on three major functions: Disease Surveillance, Case Investigation, and Intervention. The section is led by the Chief and the Deputy Chief and is composed of 4 branches: Investigations, Analytics and Informatics, Immunization, and Administration. We are a large, skilled, passionate team of ~100 FTEs that oversees the 65+ reportable diseases and conditions (excluding TB, STD and HIV) from healthcare professionals and laboratories in King County. Some of our work includes investigating cases and identifying other exposed persons requiring follow up; analyzing disease report data and trends to identity opportunities for disease prevention; monitoring emergency department visits for indications of outbreaks or biological terrorism; conducting influenza surveillance year-round. We also lead immunization strategies to increase vaccine uptake especially among school age children and high-risk groups. Cross-collaboration across teams is central to our workflow and each team supports one another to achieve the objectives of the section.</div><div></div><div><b><i><br>

Develop, inform, and distribute guidance documents and trainings to partners, including internal program leads, Public Health leadership, Washington State Department of Health (DOH), and community-based organizations. Participate in and present educational webinars and other outreach activities as needed.</li><li>
Provide technical assistance, education, and outreach to health care facilities and health care providers regarding the PHBPP, the birth dose of hepatitis B vaccine, benefits of screening, and prevention of perinatal transmission of hepatitis B infection.</li><li>
Represent Public Health with internal and external partners, including community and academic partners, coalitions, other local health jurisdictions, and the DOH.</li><li>
In collaboration with other team members, monitor performance and develop and refine program activities and goals, implementing changes as needed.</li><li>
With support from the Adult Immunization Team Lead and Best Starts staff, develop and disseminate program evaluation and data summaries about outreach and education activities to CD-Imms Section managers, community partners, healthcare providers, and the public.</li><li>
Facilitate team meetings to coordinate PHBPP activities within and across teams.</li><li>
Coordinate with other Best Starts staff as appropriate.</li><li>
Under the direction of the Adult Immunization and Vaccine Preventable Disease Team Leads, manage and track Best Starts and DOH grant deliverables related to outreach and education activities.</li><li>
