A typical week includes:</p><ul><li>In-home and community field visits &mdash; you go where your members are, in their homes and at their clinics, building the kind of relationship no clinic appointment allows</li><li>Health Action Plan development &mdash; with each member, you build a personalized plan addressing chronic conditions, goals, and the real-world barriers between them and better health</li><li>Cross-provider coordination &mdash; you are the connective tissue between the member's doctors, behavioral health providers, housing supports, and social services</li><li>Telehealth visits and phone outreach &mdash; Tier 2 and Tier 3 visit types in addition to in-person field work</li><li>Accurate, timely documentation in OneHAP and Lead Organization systems &mdash; we train you completely on every system before you see your first member</li></ul><p><b>Backgrounds We Specifically Want to Hear From</b></p><ul><li>Community Health Workers (CHWs) &mdash; especially those with WA DOH Core Competency certification. You qualify directly. We just need to build the documentation.</li><li>Certified Medical Assistants and Registered Medical Assistants with an Associate's degree &mdash; your clinical foundation is exactly what Health Home care coordination needs, especially with field, FQHC, or community health experience.</li><li>Certified Peer Support Specialists (CPSS) &mdash; lived experience with behavioral health and substance use is exactly what our member population needs. Your credibility in the community is a genuine asset.</li><li>Housing navigators and case aide workers &mdash; if you have been doing home visits, case files, and resource navigation in any social services setting for 3+ years, your experience translates directly.</li><li>Bilingual and bicultural community advocates &mdash; Spanish, Somali, Vietnamese, Amharic, and other language communities are heavily represented in our member population. Bilingual CCs are exceptionally high-value and in short supply.</li><li>Recovery coaches and substance use outreach workers &mdash; your lived experience and community trust are not entry-level.
