Primary concentration will be on patient interaction and assisting the Nurse Practitioner/Physician Assistant (NP/PA) with Population Health Visits in the home, provider offices, or via Telehealth. The candidate will have strong phone communication and computer skills. Concentration may also include follow up on patient care, scheduling Population Health Visits, and assisting with Population Health initiatives.Essential Duties &amp; Responsibilities:<ul><li>Gather patient information such as medical/surgical history, family history, risk factors, vital signs, and height/weight during the Population Health Visit and/or via telephone prior to the visit</li><li>Perform screenings during the visit such as IRIS eye exam, PVD screening, HgbA1C, urine microalbumin, etc.</li><li>Travel with providers in clinics and to patient homes to assist with triaging and EMR (Electronical Medical Records) documentation</li><li>May include traveling alone to patients&rsquo; homes to perform hybrid Population Health Visits. Hybrid visit duties include taking vital signs, performing screenings, gathering patient information, documenting in the EMR, and connecting the patient to a Population Health NP/PA via Affinity Health Group&rsquo;s (AHG) approved live video platform for completion of the visit</li><li>Calling to schedule Population Health Visits</li><li>Provide patient education regarding importance of preventive screenings and other healthcare needs that may arise with the visit</li><li>Coordinate and schedule screenings for patients such as mammogram, colonoscopy, provider appointments, etc.</li><li>Coordinate patient care with needed resources such as Integrated Care Management, Social Services, Case Management, Disease Management, etc.</li><li>Provide follow up phone calls for AHG patients to improve healthcare outcomes.  Patient Care Technician
 
 supports the work of the Registered Nurse to meet the needs of patients in order to provide safe, efficient, timely and excellent high quality care.
 
 
 
 Essential Duties And Responsibilities
 
 
 
 <ul>
 <li>
 Answer patient call signals, signal lights, bells, or intercom systems to determine patients' needs.
 </li>
 <li>
 Turn or reposition bedridden patients.
 </li>
 <li>
 Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, standing, walking, or exercising.
 </li>
 <li>
 Review patients' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet.
 </li>
 <li>
 Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff.
 </li>
 <li>
 Record vital signs, such as temperature, blood pressure, pulse, or respiration rate, as directed by medical or nursing staff.
 </li>
 <li>
 Gather information from caregivers, nurses, or physicians about patient condition, treatment plans, or appropriate activities.
 </li>
 <li>
 Observe or examine patients to detect symptoms that may require medical attention, such as bruises, open wounds, cognition, abnormal vital signs, any signs of distress or blood in urine.
 </li>
 <li>
 Document or otherwise report observations of patient behavior, complaints, or physical symptoms to nurses.
 </li>
 <li>
 Remind patients to take medications or nutritional supplements.
 </li>
 <li>
 Comply with the hospital&rsquo;s mission and values.
 </li>
 <li>
 Take an active role in the importance of rounding.

 High school diploma or GED preferred
 </li>

 Qualifications/Experience
 </li>
 <li>
 Six (6) months of experience, within the last three (3) years, in a hospital, clinic or in a SNF/Home Health setting as a nurse assistant, care partner, medical assistant or student nurse with one semester in a clinical setting.
 </li>
 <li>
 Current Basic Life Support (BLS)
 </li>
 <li>
 Must complete annual Workplace Violence Prevention Program/Certificate, per hospital policy, during initial training/orientation but not to exceed 30 days from hire/transfer.
 </li>
 <li>
 Current California State Nurse Assistant certification preferred
 </li>
 <li>
 A solid understanding of human anatomy, basic medical terminology, and rehabilitation principles is desired.
 </li>

<div>Position Summary
The Patient Care Technician (PCT) supports the work of the Nursing staff to meet the needs of patients in order to provide safe, efficient, timely and excellent high quality care. Primary duties include taking and recording vital signs, assisting patients with hygiene and elimination, ensuring the safety of the patient, and maintaining a clean and safe unit environment. Assists with stocking unit supplies, operating hospital equipment as directed, performing routine clerical tasks, and assisting with other related duties as assigned. Supports the mission, vision and goals of the organization and meets compliance with regulatory agencies. Demonstrates behavior consistent with the core values and supports the hospital plan and goals of the organization.
Essential Job Functions / Major Areas of Responsibility

<ul><li>REPORTING AND DOCUMENTATION OF CARE: Accurately measures and records patient weights and vital signs. Recognizes and reports changes in the patient's condition, takes appropriate action and accurately documents all activities as required. Receives and accurately inventories and secures patient belongings as appropriate
</li><li>PATIENT COMFORT AND PRIVACY: Assists patient with feeding, hygiene, elimination, routine skin care, comfort measures and other Activities of Daily Living (ADL), in consideration of patient age, gender, culture and stated preferences, maintaining patient privacy and respect at all times. Assumes responsibility for maintaining a cleansafe environment. Ensures Infection Control procedures and HIPAA compliance.
</li><li>COLLABORATION FOR PATIENT CARE: Assists with patient care activities as directed by licensed nurse, including specimen collection/transport, turningpositioning, and patient transportation. Performs frequent and hourly rounding to ensure safety and promotes positive patient experience.
</li><li>COMMUNICATION: Actively engages in caring behaviors, taking conscious ownership for work and takes a proactive, deliberate, and thoughtful actions regarding work activities. Demonstrates appropriate use of independent judgement and decision making.</li></ul>
Required Qualifications<ul><li>

AHA Basic Life Support</li><li>
