Your dedication will help ensure that patients receive dignified, respectful, and personalized care tailored to their unique needs.</p><p>Responsibilities</p><ul><li>Conduct comprehensive patient assessments to evaluate social, emotional, and environmental needs within hospice settings.</li><li>Develop individualized care plans that incorporate end-of-life care principles, behavioral therapy techniques, and community resource linkage.</li><li>Provide direct social work services including crisis intervention, mental health counseling, grief support, and family education.</li><li>Collaborate with medical professionals to facilitate discharge planning, medical documentation, and coordination of hospice services.</li><li>Assist patients and families with navigating healthcare systems such as Medicare and Medicaid while ensuring compliance with HIPAA and clinical ethical guidelines.</li><li>Collect data for clinical research initiatives related to hospice care outcomes and behavioral health interventions.</li><li>Maintain accurate medical records using EMR (Electronic Medical Records) systems while adhering to documentation standards.</li></ul><p>Qualifications</p><ul><li>Bachelor's or Master&rsquo;s degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law.</li><li>Proven experience working with geriatric patients, individuals with disabilities, dementia care (including Alzheimer&rsquo;s), or those requiring end-of-life support.</li><li>Knowledge of hospice &amp; palliative medicine practices, medical terminology, ICD coding, and discharge planning procedures.</li><li>Demonstrated skills in child &amp; family counseling, behavioral therapy techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and crisis management.</li><li>Familiarity with community resource coordination for senior care, assisted living facilities, home &amp; community care experience, and inpatient/outpatient settings.</li><li>Ability to work effectively with diverse cultural backgrounds and individuals with developmental disabilities or autism spectrum disorder (ASD).</li><li>Strong communication skills for client interaction, group therapy facilitation, case management, and clinical interprofessional collaboration.</li><li>Competency in medical documentation standards including HIPAA compliance and clinical ethical guidelines; proficiency with EMR systems is preferred.</li></ul><p>Embark on a rewarding journey where your expertise transforms lives during their most vulnerable moments.
