7. Participating in funding source reviews and make recommendations for treatment and lengths of stay. 
8. Monitor client progress and adjust treatment plans as needed. 
9. Coordinate with agency and community resources to ensure a through treatment plan. 
10. Assist clients with developing a discharge/continuing care plan and connecting to resources needed to reach personal goals both during the course of treatment and in advance to their discharge. 
11. Follow up with clients after discharge based on regulatory guidelines until they have been established with other case management/social services.</li></ul>

The Military OneSource Program provides 24/7 support services for military members and their families, similar to a commercial Employee Assistance Program (EAP). This program provides a broad array of information, resources, education, referrals, and counseling to about 4.7 million participants which includes military service members, their families, and eligible civilians at locations worldwide. The Military OneSource program serves as a &ldquo;one source&rdquo; for resource and information encompassing a comprehensive support system.
The Call Center Licensed Triage Consultant II supports complex cases and acts as the critical link for incoming and follow-up calls and chats from members and their families at our DC Call Center. 
 
This role is responsible for providing assistance, education, information, referral services, and expert consultation on various Employee Assistance Program (EAP) and work-life issues. 
 
Candidates must possess strong customer service skills, knowledge of call center operations, and maintain the highest degree of confidentiality, sensitivity, compassion, and respect for members and their families.
Job Responsibilities<ul><li>
Serve as the senior Triage Consultant, providing leadership support and guidance on complex and high-risk cases.</li><li>

In addition to providing direct program support, this individual will lead and coordinate other SETAs within the program, ensuring analytical rigor, strategic alignment, and consistency in technical evaluation and milestone development. The role requires independent judgment, initiative, and the ability to translate cutting-edge science into actionable program strategy in a fast-paced, mission-driven environment. This is a full-time position with an opportunity for remote work possible. Approximately 25% travel anticipated. 
 
Key Responsibilities:
<ul><li>Advise the Program Manager and leadership on program strategy, technical direction, and portfolio execution in mental health and suicide prevention.</li>
<li>Support program development from concept through execution, including development of technical theses, milestones, and evaluation frameworks.</li>
<li>Identify emerging scientific opportunities, technology inflection points, implementation barriers, and policy considerations and translate into actionable programmatic strategies.</li>
<li>Lead development of clear technical problem statements and program constructs suitable for ARPA-H Requests for Solutions (RFS).</li>
<li>Develop and pressure-test measurable technical milestones, quantitative evaluation frameworks, and risk mitigation strategies.</li>
<li>Anticipate scientific, operational, and portfolio-level risks and proactively propose mitigation pathways.</li>
<li>Provide independent recommendations and options to leadership, including trade-off analyses and scenario planning.</li>
<li>Perform stakeholder engagement &amp; support/lead multidisciplinary teams participating in technical discussion, working groups, and interagency engagements as needed.
</li><li>Oversee intake and technical evaluation of proposals, white papers, and submissions.</li>
<li>Assess study design, statistical approaches, feasibility, scalability, and real-world impact; develop and monitor performance metrics and data-driven evaluation frameworks.</li>
<li>Support site visits, technical demonstrations, and performer assessments.</li>
<li>Prepare executive briefings, decision memos, and portfolio updates.</li>
<li>Coordinate with business financial managers, public affairs, and contracting as required.</li>
</ul>
 
Required Qualifications:
<ul><li>Ability to obtain a Public Trust security clearance.</li>
<li>PhD in social psychology, clinical psychology, health psychology, neuroscience, biomedical engineering, or related field.</li>
<li>8-10 years of experience in mental health research, including suicide prevention.</li>
<li>Demonstrated experience leading complex research initiatives and multidisciplinary teams.</li>
<li>Demonstrated experience with mental health clinical trials design and meta-analysis.</li>
<li>Strong quantitative and statistical expertise.</li>
<li>Experience advising government leadership or supporting federal programs.</li>
<li>Exceptional written and verbal communication skills.</li>
<li>Proven ability to operate independently and manage competing priorities.</li>
<li>Ability to obtain and maintain a Public Trust (or equivalent HHS suitability determination).</li>
</ul>

JOB SUMMARY: The Clinical Care Coordinator provides assessment of consumers&rsquo; functioning and leads development and documentation of individualized rehabilitation plans based on the Recovery Model for adults who have mental illnesses.
ESSENTIAL DUTIES and RESPONSIBILITIES:<ul><li>
Provide services to consumers in accordance with the principles and practices of the ACT Fidelity Model.</li></ul>
 a. Lead the process of developing, updating, and guiding implementation of consumers&rsquo; Individual Recovery Plan (IRP), in collaboration with consumer, Team Leader and other significant parties of the ACT team.
 b. Coordinate treatment planning and discharge meetings at area hospitals to ensure a smooth transition to the community.
 c. Develop and maintain a professional and clinical relationship with area hospital staff to ensure a smooth transition for agency consumers to and from the hospital.
 d. Educate and assist consumers with learning or re-learning activities of daily living (ADL) skills necessary to improve the quality of life including acquisition of consumer health care benefits.
 e. Update Consumer&rsquo;s IRP, Functional Psychosocial Assessment (FPSA), Level of Care Utilization System (LOCUS), Global Appraisal of Individual Needs (GAINS) and Crisis Plan every 180 days.
 f. Provide an average of 4.25 hours of services per day to ACT consumers.
 g. Establish constructive relationships with consumers to promote acquisition of rehabilitative goals.
 h. Maintain adherence to all policies and procedures consistent with Dept. of Mental health (DMH) and the District of Columbia board of social work.<ul><li>
Participate in treatment team meetings and conduct follows ups as indicated in the following areas.</li></ul>
 a. Crisis assessment and intervention.
 b. Symptom assessment, management and individual supportive therapy.
 c. Assistance in the treatment of substance abuse for consumers with co-occurring addictive disorders.
 d. Psychosocial rehabilitation and skill development.
 e. Recommend and sometimes make decisions about the best course of action for a particular service user.
 f. Provide support services to ACT team and consumers.
 g. Develop IRP and other care plans as indicated. Conduct needs and risk assessment.
 i. Provide resolution and referrals services for consumers.
 j. Conduct psychosocial assessments using the Functional Psychosocial Assessment (FPSA) every 180 days for consumers served by the ACT team
 k. Education, support and consultation to consumers&rsquo; families and/or their support system, which is directed exclusively to the well-being and benefit of the consumer.<ul><li>
Develop and implement quality improvement (QI) activities for individual clinical staff members and the clinical aspects of the ACT team. Attend QI meetings as appropriate;</li><li>
Ensure that all IRP&rsquo;s, FPSA&rsquo;s, and other pertinent paperwork is submitted on time.</li></ul>
Requirements:

EDUCATION and EXPERIENCE:<ul><li>
Bachelor&rsquo;s degree in Social Work or related field.</li><li>
Current DC professional license as applicable i.e. LSWA.</li><li>
Three years of experience working with consumers in mental health setting.</li><li>
Driving is required. Must have valid local driver&rsquo;s license and clean driving record.</li></ul>
