Qualifications</b><p>

Performs routine diagnostic and therapeutic procedures according to accepted practice standards.</li><li>
Assist with assigned PCP&rsquo;s panel in managing lab results, reviewing consult reports, and reviewing consultation reports as determined by the PCP.</li><li>
May administer medications according to the same procedure and protocols applied to the nursing staff</li><li>
Assists in communicating the management of transitions of members with complex care needs. Discusses and refers complicated patients outside of clinical expertise to appropriate physician.</li><li>
Provide health education and counseling during visits.</li><li>
Provides on-call telephone coverage on a rotating schedule.</li><li>
Assists with direction of care team with commitment to all team members practicing to the top of their license</li><li>
Follow all protocols for daily huddle</li><li>
Participates in CQI activities.</li><li>
Attend patient care related meetings as necessary, including but not limited to morning report, case conferences</li><li>
Manage coordination of care for individual clients by working with the available resources both internal and external to agency</li><li>

Attends conferences and community based activities, including advocacy efforts to eliminate homelessness, HIV and disparities in care as designated by the Medical Director.</li></ul><p><b>
Education and Certification</b></p><ul><li>
Graduate of an Accredited Nurse Practitioner Program</li><li>
Licensed and Registered in New York State</li><li>
Certified by either the American Academy Nurse Practitioners or American Nurses Credentialing Center (ANCC)</li><li>
BLS Certified</li><li>
Achieves and maintains HIV specialist status</li><li>
Possess or obtain within first 90 days of employment a DATA Waiver for Buprenorphine Prescribing authority</li><li>
Accumulate a minimum of 15 HIV specific CME credits throughout the year and a minimum of 25 Credits per year to maintain certification requirements</li></ul><p><b>
Special Skills and Knowledge</b></p><p>

Bilingual preferred (English/Spanish or English/French).</li><li>
Experience in treating hard to reach populations, including the homeless, immigrants, HIV infected, substance users, and mentally ill.</li><li>

Leadership abilities with trouble shooting and problem solving skills.</li><li>
Proficient in use of electronic medical record systems and other technology required for use in the delivery of care.</li><li>
Track record of positive interpersonal skills with staff, clients, and others.</li><li>
Willingness to work off hours on special projects as reasonably dictated by Medical Director.</li><li>
The spirit and desire to-practice in a community based interdisciplinary, mission driven system of care.</li></ul><p><b>
Special Requirements</b></p><ul><li>
The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.</li><li><i>
