We are seeking an experienced Telehealth Nurse Practitioner (NP) to join our Mobile Acute Care Program. This fully remote role provides direct virtual patient care while also supporting our mobile field clinicians through timely EHR inbox management, clinical coordination, and transitions of care.

Position Summary</h2>
The Telehealth Nurse Practitioner will provide virtual evaluation and management of patients with acute, urgent, and follow-up care needs. The NP will conduct telehealth visits, perform virtual assessments, develop diagnostic and treatment plans, prescribe medications within the scope of practice, and provide transitional care management following hospital or facility discharge.

Required Qualifications</h2><ul><li>
Active, unrestricted Nurse Practitioner license in good standing.</li><li>

Experience providing care in mobile medicine, home-based care, hospital-at-home, urgent care, emergency medicine, or transitional care settings.</li><li>
Experience with Transitional Care Management (TCM).</li><li>

Licensure &amp; Multi-State Coverage</h2>
An active Texas NP license is preferred for this position. Multi-state licensure is strongly preferred and may expand the ability to provide telehealth services across CHP's growing service area.
The selected candidate must be willing to obtain additional state licensure, as directed and supported by CHP, to meet program growth and patient coverage needs.
Candidates are responsible for maintaining all required licenses, certifications, registrations, and credentials necessary to practice within each applicable jurisdiction.<h2 class="jobSectionHeader">
