We are seeking an experienced Telehealth Nurse Practitioner (NP) to join our Mobile Acute Care Program. This fully remote role provides direct virtual patient care while also supporting our mobile field clinicians through timely EHR inbox management, clinical coordination, and transitions of care.
The ideal candidate is a confident, clinically sound Nurse Practitioner who thrives in a fast-paced environment, demonstrates excellent independent judgment, and is comfortable managing acute and urgent patient needs through telehealth.<h2 class="jobSectionHeader">
Position Summary</h2>
The Telehealth Nurse Practitioner will provide virtual evaluation and management of patients with acute, urgent, and follow-up care needs. The NP will conduct telehealth visits, perform virtual assessments, develop diagnostic and treatment plans, prescribe medications within the scope of practice, and provide transitional care management following hospital or facility discharge.
In addition to direct patient care, this position will play an important role in supporting the Mobile Acute Care team by managing the EHR inbox, reviewing clinical messages and diagnostic results, responding to refill requests, and coordinating patient escalations and care plan updates with field clinicians.
This position will initially operate Monday through Friday during standard business hours, with weekend coverage anticipated as the program expands.<h2 class="jobSectionHeader">
Key Responsibilities</h2><ul><li>
Conduct telehealth visits with patients using a secure, HIPAA-compliant video and audio platform, including acute, urgent, and follow-up visits.</li><li>
Obtain comprehensive virtual patient histories and perform appropriate virtual assessments.</li><li>
Evaluate symptoms, establish differential diagnoses, and develop appropriate diagnostic and treatment plans.</li><li>
Conduct Transitional Care Management (TCM) visits for patients following discharge from hospitals, skilled nursing facilities, rehabilitation facilities, and other care settings.</li><li>
Order, review, and interpret laboratory testing, imaging, and other diagnostic studies as clinically indicated.</li><li>
Prescribe medications within the NP's scope of practice, state licensure, and applicable CHP clinical protocols.</li><li>
Manage the Mobile Acute Care team's EHR inbox, including clinical messages, patient updates, diagnostic results, refill requests, and other incoming tasks.</li><li>
Respond to clinical inquiries and EHR tasks in a timely manner consistent with CHP standards and patient care priorities.</li><li>
Collaborate closely with mobile field clinicians regarding patient escalations, changes in clinical status, care plan updates, and transitions of care.</li><li>
Identify patients who may require an in-person evaluation and coordinate appropriate escalation to the Mobile Acute Care team.</li><li>
Maintain timely, accurate, and complete documentation of all patient encounters and clinical activities in Athena EHR.</li><li>
Ensure documentation meets clinical, billing, regulatory, and compliance requirements.</li><li>
Participate in case reviews, quality improvement initiatives, clinical meetings, and protocol development as the program grows.</li><li>
Maintain a professional, private, and HIPAA-compliant workspace suitable for remote clinical practice.</li><li>
Follow CHP policies, clinical protocols, quality standards, and applicable federal and state regulations.</li></ul><h2 class="jobSectionHeader">
Schedule</h2><ul><li>
Monday&ndash;Friday initially, during standard business hours.</li><li>

Required Qualifications</h2><ul><li>
Active, unrestricted Nurse Practitioner license in good standing.</li><li>
Family Nurse Practitioner (FNP) certification required.</li><li>

At UT MD Anderson, the Central Nervous System Advanced Practice Provider collaborates with physicians, nurses, and multidisciplinary specialists to optimize outcomes for patients with brain and spine tumors. The department focuses on comprehensive care before, during, and after radiation therapy at the Texas Medical Center Main Campus. 
 
The ideal candidate for this role is a compassionate, team-oriented clinician with education and licensure appropriate for advanced practice provider practice. Preferred experience includes at least one year as an inpatient advanced practice provider or two years in an outpatient clinical setting.
