The CareConnect Team at WellBe supports timely response to patients&rsquo; changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner.</div><div>
This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum.</div>
The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. PRIMARY RESPONSIBILITIES<ul><li>
