Requirements 
<ul><li>
Master&rsquo;s degree in Nursing from an accredited Nurse Practitioner program</li><li>
Active and unrestricted Advanced Practice Nurse (APN) license in respective state</li><li>
Current DEA registration and prescriptive authority (or eligibility)</li><li>
Board certification as a Nurse Practitioner (e.g., FNP, AGNP)</li><li>
2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settings</li><li>
Strong clinical assessment, diagnostic, and treatment planning skills</li><li>
Knowledge of chronic disease management and geriatric care principles</li><li>
Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizations</li><li>
Proficiency in evidence-based practice and clinical decision-making</li><li>
