Summary: 
 
The scope of practice for the Ambulatory Clinical Pharmacist Specialist on the Rush at Home team focuses on chronic disease management for a patient population that faces challenges making into the traditional clinic environment. The pharmacist will be an embedded member, along with a physician/APP and nursing staff, of the Rush at Home team. The pharmacist will be responsible for providing patient care according to health system procedures and physician/APP-pharmacist collaborative agreements. Dependent on individual patient needs, the pharmacist will go to patients&rsquo; homes with the other members of the team and/or follow-up independently with patients via telehealth. On days where the pharmacist is not in the community with the team, they will be able to work remotely to complete telehealth visits.
Additionally, the pharmacist will be expected to contribute to and participate in quality improvement initiative for other community-based Rush programs. The pharmacist will have protected time to work on these other projects.

Required Job Qualifications: 
 
<ul><li>Bachelor of Science in Pharmacy.
</li><li>Illinois Pharmacist License.
</li><li>Minimum of four years of Clinical Pharmacy experience OR PGY 1 residency completed within the last seven years, which includes Clinical / Specialty Pharmacy experience.
</li><li>Knowledge in intravenous admixture services, intravenous drug therapy and chemotherapy.
</li><li>Must be able to work effectively with others to provide safe and effective drug therapy.
</li><li>Excellent problem-solving skills and the ability to work independently and quickly in responding to and resolving a wide range of problems.
</li></ul> 
Preferred Job Qualifications:
 
<ul><li>BLS Certification.
</li><li>ACLS Certification (or shall be obtained upon hire).
</li><li>Board of Pharmaceutical Specialties Board Certification.
</li><li>Doctor of Pharmacy degree and residency certificate.
</li><li>Experience and strong knowledge base in specialty drug therapies.
</li></ul>Responsibilities: 
 
<ul><li>Complete in-person visits in patients&rsquo; homes as part of a multi-disciplinary team to promote health equity and develop comprehensive care plans for high-risk patients
</li><li>Conduct scheduled telehealth appointments with patients to address healthcare and medication needs
</li><li>Participate and lead quality improvement initiatives for other community-based Rush program as needed
</li><li>Develop and implement chronic disease management standards for clinical pharmacy staff in the ambulatory setting
</li><li>Serve as a liaison between the pharmacy department and other clinic staff as needed
</li><li>Participate in departmental and health system committee and workgroup meetings
</li><li>Clarify prescriber orders, when necessary, through direct and timely contact with the prescriber or designated practitioner
</li><li>Provide patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff
</li><li>Participate in the pharmacy department's current documentation methods for clinical interventions
</li><li>Implement all drug use policies and formulary decisions of the Pharmacy and Therapeutics Committee
</li><li>Participate in safety event (ADR and medication error) reporting programs
</li><li>Precept pharmacy students and residents (PGY1 and PGY2) in assigned clinical areas
