Position Overview</b></p><p>
Under the supervision of the Site Medical Director, Nurse Practitioners will deliver comprehensive, compassionate, and thoughtful care to patients with complex chronic diseases within the New York City jail system. This role involves collaboration with physician leadership to provide general primary care, which includes conducting detailed histories and physical exams, diagnosing and managing both acute and chronic illnesses, and assessing the necessity for consultative services. In addition, the Nurse Practitioner will engage as part of an interdisciplinary team, focusing on patients with chronic medical conditions or physical disabilities who do not require acute inpatient admission. This approach ensures a holistic view of patient care and leverages the expertise of diverse healthcare professionals to optimize treatment outcomes.</p><p></p><p><b><br>
Responsibilities include: (Detailed Task)</b></p><br>

Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated.</li><li>
Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable.</li><li>
Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits.</li><li>

A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient's condition that falls within their specialty area of practice.</p>

Responsibilities</b><p>
CLINICAL PRACTICE</p>
<p>
1. Assesses the health status of patients/families by means of comprehensive health history and physical examination.</p>

2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines</p>

Qualifications</b><p>

State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows:</p>

b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course).</p>

c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner.</p>

Education:</p>

Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient's condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate.</p>
