Physical Therapist (PT) Job Responsbilities:
<ul>
<li>Planning, provision and coordination of Physical Therapy to Patients of the Agency.</li>
<li>Evaluating the Patient&rsquo;s level of physical functioning and to work with the Patient/Family to provide training and/or assistive devices to eliminate or reduce areas of deficit, usually associated with gross motor disorders and functioning.</li>
<li>Work with other professional staff to establish rehabilitation and maintenance goals and activities.</li>
</ul>
Physical Therapist (PT) Job Duties:
<ul>
<li>Evaluates the Physical Therapy needs of the Patients and provides Physical Therapy Services for Patients upon receipt of an Authorized Medical Practitioner&rsquo;s Order for evaluation and treatment of a physical disability, and submits written initial evaluation for inclusion in the Patient&rsquo;s record.</li>
<li>Performs Clinical tests to discover functional limitations and the potential of restoration and, in accordance with Physician&rsquo;s orders, develops a Plan of Care with restorative and maintenance goals to meet identified needs.</li>
<li>Instructs Patients, Families, Caregivers, and other health team personnel in the use of assistive modalities and appropriate Physical Therapy procedures.</li>
<li>Observes, records and reports to the Authorized Medical Practitioner and Agency Personnel, the Patient&rsquo;s reaction to treatment, and any changes in the Patient&rsquo;s condition.</li>
<li>Works with the health team to shorten the Patient&rsquo;s recovery from illness and incorporate the management of residual disability.</li>
<li>Communicates with the Nurse at least every two (2) weeks to discuss the Patient&rsquo;s progress. Telephones the Agency immediately with any information that will influence or change the provision of services including any unusual situations observed during the course of service.
</li><li>Re-evaluates patient as needed to detect progress attained and completes a written progress report for the Patient&rsquo;s record containing Patient&rsquo;s progress, continues treatment plan, goals, frequency of visits, and outcomes of care provided.</li>
<li>Identifies problems with the operation and safety of equipment laces in the Patient&rsquo;s home and insures follow-up by the appropriate vendor and/or notifies the Agency to follow-up.</li>
<li>Submits Discharge summaries at the completion of services that includes outcome of care provided, equipment ordered and any recommendation for follow-up care.</li>
</ul>
Physical Therapist (PT) Minimum Education:
<ul>
<li>Has graduated from a Physical Therapy curriculum approved by the American Physical Therapy Association or</li>
<li>the Council on Medical Education of the American Medical Association.</li>
</ul>
Physical Therapist (PT) Minimum Experience and Skills:
<ul>
<li>A minimum of one (1) year of experience as a Physical Therapist in an Acute Care setting, two (2) years preferred, Home Care Experience desirable.</li>
<li>Demonstration of considerable knowledge in the current standards, procedures and techniques of Physical Therapy.</li>
<li>Outstanding assessment and technical skills.</li>
<li>Organized, open culturally.</li>
</ul>
Physical Therapist (PT) License/Registration/Certification/Permit Required:
