Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).<h1 class="jobSectionHeader">
PRINCIPAL DUTIES AND RESPONSIBILITIES:</h1>
Patient Related
Education:<ul><li>
Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.</li></ul>
Treatment:<ul><li>
Welcome assigned patients and inquire as to their wellbeing since their last treatment.</li><li>
Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.</li><li>
Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment</li><li>
Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
</li><li>Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
</li><li>Monitor patients&rsquo; response to dialysis therapy.</li><li>
Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
</li><li>Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing &amp; sitting blood pressure</li><li>
Obtain Hemostasis and apply appropriate dressings.
</li><li>Evaluate the patient prior to discharge.</li><li>
Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
</li><li>Responsible for calculating and entry of individual patients&rsquo; dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
</li><li>Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
</li><li>Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
</li><li>Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
</li><li>Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.</li><li>
Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.</li><li>
Perform and document any intervention for unusual patient status and document patients' response to intervention.</li><li>
Evaluate patients&rsquo; vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.</li><li>

Delivers and maintains on-going competency evaluations/audits to ensure ongoing performance of other PCT&rsquo;s.</li><li>

Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate, and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).</li><li>
Initiate Solution Delivery System (SDS) system.</li></ul>
RECORD KEEPING:<ul><li>
Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.

</li><li>Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure omitted entries are completed or corrected by appropriate staff.

</li><li>Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
</li><li>Collect, label, appropriately prepare and store lab samples according required laboratory specifications.

Assists with facility QAI infection control audits.</li><li>
Acts as a change agent for infection control practices within clinic. Under the direction of the CM or designee monitors staff and patient adherence to infection control practices.

Supports ongoing staff and patient engagement on infection control practices under the direction of CM or designee for example participation in patient lobby days and staff meetings to reinforce infection control practice.</li><li>

Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care

Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
</li><li>Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
</li><li>Sets and preforms a treatment on all machines used in the transitional care unit.
</li><li>Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.</li><li>
Assists with coordination of transition to patient&rsquo;s modality choice.

Position Summary: 
The Handy Staff plays a central role in advancing Help@Home&rsquo;s mission to ensure older adults can remain independent, safe, and comfortable in their own homes for as long as they choose. This position provides high-quality minor home repairs, accessibility modifications, and safety assessments while applying aging-in-place principles, universal design, and a client-centered approach. The Handy Staff is expected to hold, or be willing to obtain, the Certified Aging-in-Place Specialist (CAPS) designation to ensure all services reflect best practices in home safety and accessibility for older adults.
The Handy Staff supports the Aging Operations Manager in maintaining partner contracts through assessments, documentation, and relationship management. Working in close collaboration with call center staff and the social work team, the Handy Staff ensures that repairs and home maintenance projects are well coordinated, scheduled efficiently, and aligned with each client&rsquo;s care plan.
Handy Staff provide services directly in clients&rsquo; homes as scheduled by call center staff, delivering repairs and modifications with professionalism, dignity, and respect. JFS will supply all tools necessary and provide mileage reimbursement for job related travel. Through skilled work, compassionate interaction, and CAPS informed safety practices, the Handy Staff strengthens Help@Home&rsquo;s mission to support healthy aging and peace of mind for older adults in the community.
Requirements:

Essential Duties: 
<ul><li>
Complete a wide range of basic home maintenance tasks for older adults, including repairing doors, steps, flooring, lighting, general plumbing, drywall, and other light carpentry needs to support safe and comfortable living environments.</li><li>
Install essential safety features such as grab bars, handrails, nonslip solutions, improved lighting, and other fall prevention measures that enable clients to age in place with confidence.</li><li>
Evaluate client homes to identify potential hazards, recommend appropriate repairs or modifications, and prioritize work that enhances safety, accessibility, and overall wellbeing.</li><li>
Engage with older adults in a respectful, patient, and clear manner, explaining repair needs, outlining work plans, and ensuring clients feel informed and comfortable throughout the process.</li><li>
Complete job reports including work, materials used, time spent, safety concerns, and follow-up needs in accordance with organizational procedures and reporting requirements.</li><li>
Collaborate with volunteers, contractors, and community partners when additional expertise or support is needed to complete repairs or modifications.</li><li>
Adhere to all safety protocols, use tools and materials responsibly, and ensure all work meets JFS quality expectations and regulatory guidelines.</li><li>
Serve as an ambassador of the JFS mission, demonstrating professionalism, empathy, and integrity in interactions with clients, families, colleagues, and community stakeholders.</li><li>
Coordinate regularly with the JFS Older Adult Care Management Team and participate in interdisciplinary meetings as needed to ensure holistic support for clients&rsquo; safety and wellbeing.</li><li>
Document and relay observations related to client safety, functional changes, or environmental concerns that may require follow-up from the care team.
</li><li>Serve as backup support for scheduling home assessments, repair visits, and follow-up appointments when primary staff are unavailable.</li><li>
Assist with outreach, education, and community workshops related to home safety, fall prevention, and aging in place.</li><li>
Assist with JFS facility projects as needed such as minor paint touch ups, hanging items on walls, installing small shelves or equipment, and other jobs that are not handled by the property management team.</li><li>

Essential Education, Knowledge, and Experience:<ul><li>
Background in construction or trade is desired but not required. Proficiency in basic repairs is required, such as:
</li><li>Electrical repairs and maintenance like lightbulbs, wiring, circuit breakers, fuses
</li><li>Carpentry &amp; General Construction: woodwork, sanding, painting, etc.</li><li>
General Plumbing: clogged toilets, faucets, fix leaks, etc.
</li><li>Facility Repairs: doors, doorknobs, locks, replacing windowpanes
</li><li>Empathy: must have and maintain a high level of care and concern for the experiences and circumstances of our clients.
</li><li>Customer Service: demonstrated desire to serve clients by focusing effort on meeting their needs, understanding their concerns, and seeking to build trust.
