Job Description Summary: 
Through the use of evidence-based practices such as harm reduction, housing first, and trauma-informed care, the Case Manager supports Clients’ housing stability, self-sufficiency, and community integration. The Case Manager partners with Clients who are currently homeless to assess and address barriers to housing placement. Once Clients locate best-fit housing, The Case Manager works to implement interventions with recently housed Clients to support housing retention and further coordinates multiple services to meet individualized needs. The Case Manager collaborates with Clients to learn independent living skills and other life-skills vital to successful transition from homelessness into permanent housing.
 
Job Description Essential Duties and Responsibilities: 
<ul>
<li>Maintain a caseload of approximately 20 high-acuity Clients for intensive case management services (up to 40 if low acuity).</li>
<li>Develop relationships with Clients with a focus on facilitating independence and maintenance of improved physical and mental health.</li>
<li>Collaborate with the LA County Department of Mental Health (DMH) and Department of Health Services (DHS) programs to enhance coordination within the Clients’ treatment team.</li>
<li>Engage Clients and implement interventions with an approach emphasizing Client choice, harm reduction, and psychosocial rehabilitation within a setting guided by trauma-informed care, housing first principles, and professional boundaries.</li>
<li>Assist in the screening, intake, and enrollment of Clients including orientation to program policies and goals.</li>
<li>Complete comprehensive biopsychosocial assessments and functional needs assessments to guide individualized treatment planning.</li>
<li>Complete quarterly collaborative goal plans with Clients with the purpose of targeting areas that will improve quality of life and health outcomes.</li>
<li>Write and maintain accurate, complete, and up-to-date progress notes and data in electronic databases as required by the program and its funding sources.</li>
<li>Ensure all Client documents within physical charts are complete, accurate, current, and filed as guided by agency, department, and funder expectations.</li>
<li>Maintain confidentiality of Client information per HIPAA and internal agency guidelines.</li>
<li>Assist Clients in applying for and obtaining all benefits to which they are entitled.</li>
<li>Coordinate appointments, transportation, and follow-up services for Clients to improve access to primary health care, mental health care, substance use services, and other local resources in order to build sustainable community connections and reduce the need for emergency health care services.</li>
<li>Directly transport Clients in personal vehicle to various agencies and healthcare providers in the greater Los Angeles area to decrease barriers to access and increase community reintegration.</li>
<li>Support Clients by teaching and modeling life skills involved such as budgeting, housekeeping, meal preparation, appointment scheduling, health maintenance, and socialization within communities where they become housed.</li>
<li>Communicate effectively with property managers and other third party partners.</li>

We are looking for a highly motivated full-time Lead Care Manager who is willing to think creatively and empathically to help our team change the way people access healthcare.

We seek a full-time Lead Care Manager to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide. 

This position primarily allows for remote work; however, it includes 2-3 times a month on-site visits in the community alongside a fellow PairMate. You can expect to engage in these in-person activities 2-3 times per month, close to your city, while the majority of your duties, approximately 90%, will be performed from your home

What You'll Do</h3>
<ul><li>Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals</li>
<li>Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system</li>
<li>Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members</li>
<li>Work with member to identify health/wellness goals and incorporate goals into Health Action Plan/Shared Care Plan</li>
<li>Supports nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with delegated tasks</li>
<li>Collaborates on care issues with Enhanced Care Management team by participating in systematic case reviews and consulting with nurse care manager, behavioral health care manager, and nurse practitioner before taking clinical actions</li>
<li>Consistently meet monthly encounter metrics to ensure compliance with health plan regulations</li>
<li>Identify and break down barriers ensuring individuals' continuation with the program</li>
<li>Assists individuals in securing connection to community supports by scheduling appointments, managing referrals, and ensuring timely follow-ups</li>
<li>Coordinate physical care management appointments through collaboration with external and internal providers</li>
<li>Utilize external and internal online platforms to collaborate with team members and carry out daily tasks</li></ul>

<ul><li>1+ years of general work experience (Case Management preferred)</li>
<li>You are physically located in California
<ul><li>Field Ops requires you to maintain reliable transportation for engagement at clinic, community based organization, and health system partner locations</li>
<li>Virtual Ops requires a quiet, HIPAA compliant and internet connected space</li>
