Position Description: 
The Case Manager will provide a broad range of case management services to children in substitute care and the families with whom they are placed. Primary responsibilities include prioritizing and managing crisis frequently; working one on one with client, working with larger groups such as families, staff groups and focused task groups; maintaining a high load of telephone communication; and handling after-hours on-call when scheduled. 
 
Primary Responsibilities:
<ul><li>Provide intake and assessment activities that will assist clients and their family in determining which services are most appropriate for them in developing an individualized service /treatment plan.</li><li>
Utilizing client and family input, develop a plan of service that selects and outlines an array of services and interventions from within DePelchin and the community that will address the needs formulated in the assessment process.</li><li>
Assist clients with referrals to other community services identified in the plan of service and provide advocacy for individual clients and their families to receive entitlements or needed services.</li><li>
Provide support services in anticipation of discharge and post-discharge services that are most appropriate for them to aid the transition to a new placement, track outcomes for youth in care, support normalcy.</li><li>
Support the family and client post-discharge through Aftercare service delivery.</li><li>
Provide crisis management support for children and families by implementing interventions that support de-escalation.</li><li>
Complete quarterly inspections for assigned homes.
</li><li>Maintain contact with foster families, to ensure compliance.</li><li>
Follow all agency/department/program policies and procedures and comply with the legal and regulatory requirements of the Council on Accreditation, Youth for Tomorrow, Residential Child Care Licensing, Residential Child Care Contracts and other appropriate regulatory bodies.</li><li>
Enter relevant information/changes and renewable information into Solutions.</li><li>
Serve as liaison to TDFPS Residential Child Care Licensing for sampling visits and other related inspections for the foster/adoptive home.</li><li>
Ensure all documentation is entered into electronic health record. Monitor frequently to ensure facility and/or client compliance.</li><li>
Maintain current child record meets all recording requirements of the agency.</li><li>
Provide monitoring of foster home specific to children&rsquo;s plan of service and supervision needs as well as compliance with the physical home in accordance with Minimum Standards.</li><li>
Coordinate treatment by consulting and collaborating with referral sources, teachers, school counselors, and other professionals involved with the client.</li><li>
Meet productivity expectations by maintaining assigned caseload and managing schedule to meet appropriate timeliness.</li></ul>
Required Qualifications:<ul><li>
Bachelor&rsquo;s degree, preferably in social work, or other human services field.</li><li>
Two (2) years of full-time documented experience in child welfare services conducting assessments, service planning, case management, and crisis management preferred.</li></ul>

 The Continuum of Care (CoC) Program Case Manager position is responsible for assisting homeless individuals and families obtain housing and providing services and resources to promote long-term residential stability.
 
 
 
 Duties And Responsibilities

 Accepts Coordinated Access System referrals as assigned and follows established protocols to enroll clients into housing programs.

What You'll Do

As a Behavioral Health Care Manager (BHCM) with Imagine Pediatrics, you will work with the families of medically complex children providing case management services in accordance with Case Management Society of American (CMSA) Standards of Practice for members enrolled in Imagine Pediatrics behavioral health program. You will work alongside pediatricians, nurses, care coordinators, and other healthcare professionals. Your primary responsibilities will include:

<li>Monitor high-risk pediatric patients (up to 19 years old), some recently discharged from the hospital, ensuring appropriate follow-up and clinical management, and adjusting care plans as needed.</li>
<li>Conduct biopsychosocial assessments to address behavioral, social, emotional, and systemic needs of the patient and family.</li>
<li>Create and evaluate the effectiveness of the patient/family's care plan and modify based on families evolving needs and goal progression.</li>
<li>Provide intervention that is consistent with the social/emotional/physical needs of patients and caregivers such as mental health crises, behavioral issues, and family conflict.</li>
<li>Facilitate case management and support that requires clinical expertise in various systems with focus on helping patients and families negotiate the complexities involved with a mental health diagnosis.</li>
<li>Resource validated external services requested by the family to meet behavioral and social needs such as social services agencies and behavioral specialists.</li>
<li>Provides interventions in response to crisis to de-escalate and stabilize patient and family members</li>
<li>Provides psychoeducation on the nature of mental health diagnosis and progression, the importance of treatment adherence, and related information as appropriate</li>

The Requirements We are Seeking:
<ul><li>Bachelor&rsquo;s Degree in Social Science, Psychology, Counseling, Sociology, Social Work, Criminal Justice, or Public Health</li><li>
A minimum of one year of experience working with survivors of domestic and sexual violence, or social services setting working with vulnerable populations with clients from diverse backgrounds.
</li><li>Intermediate Proficiency within Microsoft Office, specifically Teams, Outlook, Excel, and Word.</li><li>
Bilingual in both English and Spanish, preferred.</li><li>
You have a valid Texas driver&rsquo;s license with a good driving record; reliable transportation; ability to travel to off-site locations utilizing personal transportation</li></ul>
What you Deliver in this role:
<ul><li>You will provide hospital accompaniment, crisis intervention, emotional support, case management, information &amp; referral, education, support, and advocacy to survivors of domestic and sexual violence at local hospitals and via nurse line as needed.</li><li>
You will provide crisis intervention, emotional support, case management, information &amp; referral, education, support, and advocacy to survivors of domestic and sexual violence via 24 crisis hotline and chat as needed while on standby for hospital visits.</li><li>
You will conduct danger assessment and creates safety plans with survivors in high-risk cases.</li><li>
You will communicate with on-call staff including Supervisors, Crisis Hotline/Chat Counselors, SANE nurses, law enforcement, and community partners.</li><li>
You will coordinate shelter placement and emergency transportation of clients as needed with Hotline and Shelter team.</li><li>
You will attend mandatory monthly meetings and training with Manager of Hospital Accompaniment &amp; Crisis Intervention Services, All Department Staff Meetings and/or meet as assigned.</li><li>
You will do mandatory Reporting: report suspected and prior abuse/neglect to either Child Protective services or Adult Protective Service when appropriate.</li><li>
You will maintain sufficient inventory at all times while on-shift of information packets and clothing for clients. Supplies provided by agency.</li><li>
Other duties/projects may be assigned. Should such duties become routine, the job description will be reviewed and revised.</li></ul>
