<p>
As an integrated care team member, the Medication Support Navigator focuses on improving medication related outcomes by supporting medication access needs for pediatric patients with chronic medical and behavioral health conditions. The medication Support Navigator works alongside the Clinical Pharmacist with primary responsibilities including maintenance of patient medication therapy records, accurately capturing medication histories, troubleshooting medication access barriers with community pharmacies and pharmacy benefits managers, and outreach to patient caregivers.</p>
<p>
In this role you will:</p>
<ul>
<li>Manage, triage, and assign clinical pharmacy referrals to appropriate clinical pharmacy team member</li>
<li>Support internal and external team members in the clinical pharmacy referral process based on patient needs</li>

We are currently seeking a Psychiatric Nurse Practitioner to join our team and provide medication management services as part of our telehealth platform.</p><p></p><p><b><br>

Job Description:</b><br>
<ul><li>Conduct virtual psychiatric assessments and evaluations to determine appropriate medication management plans for clients.</li><li>Prescribe and monitor psychiatric medications, adjusting dosages and treatment plans as necessary.</li><li>Collaborate with our team of licensed therapists and counselors to ensure integrated and comprehensive care for clients.</li><li>Provide education and support to clients and their families regarding medication adherence, potential side effects, and therapeutic interventions.</li><li>Maintain accurate and timely clinical documentation and adhere to ethical guidelines and legal requirements.</li></ul></div>
<ul><li>Autonomy over clinical decisions: You are the licensed and trained provider. We defer to your clinical judgement and expertise about the most effective and clinically appropriate approach for clients.</li></ul><p></p><div><b><br>
Qualifications:</b><br>
<ul><li>Master's degree in Nursing with specialization in psychiatric-mental health.</li><li>Current state licensure as a Psychiatric Nurse Practitioner and national certification.</li><li>Minimum of 2 years of post-certification clinical experience in psychiatric-mental health.</li><li>Experience with telehealth platforms and technology for virtual patient consultations.</li><li>Strong clinical assessment, diagnostic, and medication management skills.</li><li>Excellent communication, collaboration, and interpersonal skills.</li></ul></div><p></p><div><b><br>
