The Telehealth Nurse Practitioner will provide virtual evaluation and management of patients with acute, urgent, and follow-up care needs. The NP will conduct telehealth visits, perform virtual assessments, develop diagnostic and treatment plans, prescribe medications within the scope of practice, and provide transitional care management following hospital or facility discharge.</p><p>
In addition to direct patient care, this position will play an important role in supporting the Mobile Acute Care team by managing the EHR inbox, reviewing clinical messages and diagnostic results, responding to refill requests, and coordinating patient escalations and care plan updates with field clinicians.</p><p>

Conduct telehealth visits with patients using a secure, HIPAA-compliant video and audio platform, including acute, urgent, and follow-up visits.</p></li><li><p>
