Job Overview
The Home Care Coordinator is responsible for the support and implementation of home care services for participants. Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings. Responsible for acting as the liaison between the IDT and the home care providers, she or he should possess good leadership and communication skills. In addition, the Home Care Coordinator should be able to identify and understand participants&rsquo; medical and social needs to evaluate the home care needs of participants.
Minimum Requirements

POSITION SUMMARY
The Manager of Care Management will supervise and direct all levels of Care Management team that includes RN Care Managers, Lead Care Management RN, Care Management Coordinators, Discharge Planners (DCP), Care Coordinators, and Care Management Educator. This position may also be asked to supervise and cover supervision for other management in the CM Department. These services include assessment of patients for level of care, appropriateness for admission or observation, HLOC transfers, discharge assistance, and follow up care as appropriate in collaboration with the ED care management team and Social Work Department. Care Management policies for this work initiated, reviewed and implemented by the Manager as needed.
This Manager role will also provide clinical supervision for scheduling, Kronos payroll, coaching/corrective action, and educational needs to ensure that the care management policies and processes are carried out for the organization. This role will oversee the electronic referral system, currently Ensocare, documentation of care management through Cerner Healthcare EMR including working with Clinical informatics (CIT), to develop or improve care management documentation, as well as handoff processes.
This position is responsible for overseeing post-acute providers, facilities and other types of community service providers as directed by the Director of Care Management. The Manager will assist with overseeing compliance policies and plan related to vendor networks to maintain a working relationship between MLK and individual network providers. Manager will identify opportunities to improve post discharge services to reduce readmissions and improve community health in collaboration.
Works collaboratively with other colleagues within the Population Health Program to support the entire Care Management Department. This role is expected to share in Care Management Leadership Call on a rotation basis as assigned by the Director for Care Management

Three years experience in social service setting providing targeted care management to multi-ethnic communities, with knowledge of, experience with pregnant women, children and families and chronic disease management.</li><li>

ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs a combination, but not necessarily all, of the following duties:
Assessment<ul><li>
Assists with intakes and assessments of families to identify needs and care plan.</li><li>
One-on-one health care assessment and education.</li><li>
Coordinate all aspects of care; transportation, referral, scheduling, and data input where needed.</li><li>
Link patients to resources in the community.</li></ul>
Documentation<ul><li>
Care plan development including goals, objectives and actions to resolve barriers and access to services.</li><li>
Maintain client&rsquo;s files, records, appointments, services, follow-ups and assessments.</li><li>
Maintain regular contact assigned consumers; phone calls, conferences.</li><li>
Maintains accurate record keeping, client tracking, data collection and monthly reports.</li></ul>
 Screening<ul><li>
Assist in administering and interpreting screening tools.</li><li>
Continuing education and development and willing to grow and keep up with skills.</li></ul>
 Team<ul><li>
Coordinates care monitoring to follow up schedules and procedures with families.</li><li>
Maintains contact between family and service providers to monitor success.</li><li>
Attends staff meetings and open discussion forums.</li><li>
Fosters team environment in advocate role while providing social services support.</li></ul>
 Organization<ul><li>
Coordinates services and advocacy with contact to community resources</li><li>
Liaison between family and community resources.</li><li>
Strengthens the family&rsquo;s ability to assess and meet education, health and social service needs of their children.</li></ul>
