Essential Job functions:<ul><li>
Under the supervision of the Clinical Practice Manager, is responsible for planning, organizing, and coordinating daily operations to ensure quality patient care.</li><li>
Be the liaison between the Physician and staff.</li><li>
Provides leadership and support in day to day operations within the clinical setting along with the Pod Liaison, Clinical Coordinator/ Clinical Practice Manager. Serves as a role model and consultant for staff.
</li><li>In collaboration with the Clinical Coordinator/Clinical Practice Manager, ensures policies and procedures are properly communicated and adhered to. Ensures staff members are competent in emergency protocol and procedures.</li><li>
Completes and populates Physician and staff monthly schedules. Maintains the automated schedule. Manage staff PTO requests. Identifies staffing needs in advance, assists in utilizing resources effectively.</li><li>
Monitors and controls overtime use. Manages weekly staff swipe reports.</li><li>
Coordinates and assists with training, monitoring, and evaluation of staff. Plans and assists in the hiring and orientation of new office staff.</li><li>
 In collaboration with Clinical Coordinator/Clinical Practice Manager, evaluates clinical staff according to SMG policy and provides ongoing feedback. Conducts an annual performance evaluation including obtaining physician input on each staff member of the department.</li><li>
Works with QI/Practice Transformation and Clinical Practice Manager to implement and create action plans for SMG clinical quality improvement process at the POD level. Monitors all Quality dashboards.</li><li>
Monitors departmental compliance with all applicable standards (OSHA, HIPPA, Occurrence Reporting); reports compliance concerns; ensures utilization of resources for resolution of issues.</li><li>
In collaboration with Practice Transformation Department, advances the department&rsquo;s technology including the use of the information technology and the implementation and appropriate use of the Electronic Medical Record, Practice Management System, Departmental /Ancillary systems.</li><li>
Serves as liaison between Care Management and Care Coordination in support of Meaningful Use which includes but not limited to; Documentation of Information obtained during visit, Vitals, Smoking, Demographic, Patient Outreach, 21 day overdue work list, Pre-visit Planning, Outreach for Screening, Referral Tracking, Clinical Summaries and patient education material as well as Patient Portal communications.</li><li>
Works with the physician POD Liaison(s) to ensure efficient office operations. Makes adjustments in workflow as necessary to ensure overall smooth functioning of the department and compliance of SMG policies. Communicates with physicians and POD Liaison on regular basis to ensure seamless flow of communications.</li><li>
 Resource for department to manage patient issues and concerns; reports back to Providers and Clinical Coordinator/Clinical Practice Manager.</li><li>
In collaboration with Nurse Educator, maintains and expands staff competencies, as well as implementing ongoing internal education programs and encouraging participation in outside continuing education. Increases operational efficiency in PODs through cross training of personnel. Supports any/all Career Ladder opportunities to bring staff to their highest potential.</li><li>
In collaboration with Pod liaison and Clinical Coordinator/Clinical Practice Manager implements customer service action plans.</li><li>
Holds monthly POD staff meetings; communicates pertinent SMG initiatives and information to staff. Attends Clinical Practice Manager&rsquo;s meetings.</li></ul>

Compensation
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $26.8018 - $36.6767 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. Conducts monthly reviews of the medication record of each person served to monitor potential adverse reactions, allergies, interactions, contraindications, rationality and determine the need for laboratory test modifications. Advises the physician of any medication errors or issues and indicates/recommends changes.</li><li>
Reviews all consults and/or medical reports with recommendations and meets with the Residence Manager, Nursing Supervisor on a weekly basis to ensure that all follow up is completed.</li><li>
Reviews special dietary orders and ensures accurate documentation of blood pressure and weight for each person served.</li><li>
Provides first aid and daily nursing care to the people served as needed. Monitors each person&rsquo;s medical condition through observation and examination.</li><li>
Provide on-going care management and treatment of medical issues as prescribed by the Nursing Care Plans and/or physicians instructions, including but not limited to, providing soaks, changing dressings and managing G tubes and colostomy bags etc.</li><li>
Escorts/accompanies individuals on medical or psychiatric appointments as needed to ensure appropriate exchange of information.</li><li>
Participates in the weekly consult review meetings and ensures that all appointments/evaluations are completed in a timely manner.</li><li>
Responds in a timely fashion (within &frac12; hour) to all contacts from the residence requesting medical consultation/guidance and directs medical intervention until the situation is resolved. Serves as assigned on emergency medical response team for the Day Habilitation programs.</li><li>
Shares rotating on-call nursing responsibilities for after-hours and weekend medical triage and consultation.</li><li>
Ensures that the Residence Manager and staff are appropriately trained on all aspects of care for acute and chronic conditions. Provides and documents on-going staff training in the following areas:<ul><li>

 Job Summary
 
 
 
 The mission of the Patient Advocate (PaD) is to facilitate and ensure continuity in patient care and serve as a conduit of care between the services of the health center and into the community. The PaD will strategically engage employees and their dependents before, during, and after visits to the center in order to ensure we are providing personalized, informative, and compassionate care. The PaD role is supported by and reports to the Director of Patient Care Navigation. The individual will work closely with the Client Relations, Clinical Operations team, Health Center Provider, and Client HR Team.
 
 
 
 Essential Duties And Responsibilities
 
 
 
 
 Coordination of Care

 Under the direction of the Provider, the PaD will coordinate care with external specialists, imaging centers, labs, medical equipment providers, counselors, community resources, etc.

 Ensure electronic medical records are provided to Proactive MD&rsquo;s Provider following any specialist visit and that Provider records are sent to specialists prior to patient visit.
 </li>
 <li>
 Receive prior authorizations required by the insurance company for tests or procedures and, as appropriate to the Health Center, send prior notification to insurance regarding procedures or referrals.
 </li>
 <li>
 Continuously refine and update list of value-based providers suitable for referral in the community the Health Center serves.
 </li>
 <li>
 Assist patients in navigating the healthcare system.
 </li>
 <li>
 Coordinate care when a patient is admitted and discharged from the emergency room or hospital. Ensure Proactive MD&rsquo;s Provider is included in the aftercare process.

 Visit hospitalized patients to ensure care coordination is directed by the Provider.

 Coordinate with "Care Beyond the Walls RN Case Manager" with medically complex patients requiring significant out of Health Center care.

 Engage with patients to encourage use of the Health Center (high risk dashboard or patient engagement dashboard)
