In this rewarding role, you will provide personalized, high-quality in-home support services that promote independence, dignity, safety, and comfort for individuals receiving care through Taylored Care Services.As a valued member of our care team, you will assist clients with daily living activities, provide companionship and emotional support, monitor client well-being, and ensure services are delivered according to each client&rsquo;s Individual Service Plan (ISP). This position is ideal for caring, dependable, and motivated individuals who are passionate about helping others thrive in their home and community environments.Duties &amp; Responsibilities<ul><li>Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, mobility assistance, and hygiene support</li><li>Provide companionship, supervision, and emotional support to clients in home and community settings</li><li>Support meal preparation, light housekeeping, laundry, and maintaining a safe living environment</li><li>Monitor and report changes in client condition, behavior, mood, mobility, or safety concerns</li><li>Follow each client&rsquo;s Individual Service Plan (ISP) and company care guidelines</li><li>Accurately document daily services, incidents, and shift activities in electronic documentation systems such as CareSmartz360</li><li>Maintain confidentiality and HIPAA compliance at all times</li><li>Respond appropriately during emergencies and follow mandated reporting procedures when required</li><li>Assist clients with mobility support and safe transfer techniques within training guidelines</li><li>Maintain professional boundaries and respectful communication with clients, families, and team members</li></ul>Qualifications &amp; Skills<ul><li>Previous caregiving, home care, direct support, assisted living, or healthcare experience preferred</li><li>Compassionate, dependable, patient, and professional attitude</li><li>Strong communication and interpersonal skills</li><li>Ability to work independently and as part of a team</li><li>Basic computer or mobile app proficiency for documentation and scheduling systems</li><li>Knowledge of HIPAA and confidentiality standards preferred</li><li>CPR/First Aid certification preferred</li><li>Experience supporting individuals with developmental disabilities, dementia, memory care, hospice, or senior care is a plus</li><li>Reliable transportation and punctual attendance required</li><li>Ability to pass background clearance requirements</li></ul>Requirements<ul><li>Complete all onboarding, training, and required compliance requirements</li><li>Maintain accurate daily documentation and timekeeping records</li><li>Work only approved schedules and authorized assignments</li><li>Follow all safety, confidentiality, and company policies</li><li>Ability to safely perform physical caregiving tasks as required</li></ul>Benefits<ul><li>Flexible scheduling opportunities</li><li>Paid sick leave in accordance with California law</li><li>Ongoing training and professional development</li><li>Supportive team environment</li><li>Meaningful and rewarding work experience</li><li>Opportunities for career growth within Taylored Care</li></ul>Why Join Taylored Care?At Taylored Care, we believe compassionate caregivers make a lasting difference in the lives of the individuals and families we serve.
