Case Manager Job Summary</b></div><div></div><div><br>
The Case Manager is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file with supporting documentation maintained in physical files. Responsible for coordinating case management, safe and timely release services, and ensuring timely completion of ORR case management requirements. The Case Manager will facilitate coordination between all disciplines within the program to ensure timely reunification and safety of youths. The Case Manager demonstrates knowledge of and familiarity with the Office of Refugee Resettlement (ORR) policy and procedures relating to the care and supervision of unaccompanied children (UC). This includes knowledge of legal background and authority governing the practice as related to working with unaccompanied children.</div><div></div><div><b><br>
Essential Functions:</b></div><div></div><div><br>
<ul><li>
Within 24 hours of youth&rsquo;s arrival to program, complete initial intake assessment, provide initial phone calls to family in home country and sponsor, and ensure youth signs acknowledgment and receives a copy of the Legal Service Provider list and Notice to Juvenile Aliens in Federal Facilities Funded by DHS or HHS.</li><li>
Conduct sponsor assessments, home country assessments, and work in guiding sponsor to complete documents, background checks, fingerprint appointments to complete the reunification process in timely manner.</li><li>
Maintain physical and web-based client files, documents, and assessments. File all documents obtained by sponsors in youths file, along with case notes and other assessments conducted.</li><li>
Submit a completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated ORR representatives. Prepare the youth for discharge/release by reviewing and amplifying discharge plans.</li><li>
Ensure the timely completion of all assessments including Initial Intake, UAC Assessments, and Individual Service Plans in accordance with CFRC, State, and Federal requirements. Assessments are documented into appropriate web-based systems or other reporting systems as indicated by policy or procedures.</li><li>
Provide weekly face-to-face updates to youth. Ensure the provision of weekly phone calls to approved caregivers, family members, and/or sponsor and documented.</li></ul></div><div></div><div><br>

Position Description</b>:<br>
The Case Manager will provide a broad range of case management services to children in substitute care and the families with whom they are placed. Primary responsibilities include prioritizing and managing crisis frequently; working one on one with client, working with larger groups such as families, staff groups and focused task groups; maintaining a high load of telephone communication; and handling after-hours on-call when scheduled.<br>

<b>Primary Responsibilities</b>:
</p><ul><li>Provide intake and assessment activities that will assist clients and their family in determining which services are most appropriate for them in developing an individualized service /treatment plan.</li><li>
Utilizing client and family input, develop a plan of service that selects and outlines an array of services and interventions from within DePelchin and the community that will address the needs formulated in the assessment process.</li><li>
Assist clients with referrals to other community services identified in the plan of service and provide advocacy for individual clients and their families to receive entitlements or needed services.</li><li>
Provide support services in anticipation of discharge and post-discharge services that are most appropriate for them to aid the transition to a new placement, track outcomes for youth in care, support normalcy.</li><li>
Support the family and client post-discharge through Aftercare service delivery.</li><li>
Provide crisis management support for children and families by implementing interventions that support de-escalation.</li><li>
Complete quarterly inspections for assigned homes.
</li><li>Maintain contact with foster families, to ensure compliance.</li><li>
Follow all agency/department/program policies and procedures and comply with the legal and regulatory requirements of the Council on Accreditation, Youth for Tomorrow, Residential Child Care Licensing, Residential Child Care Contracts and other appropriate regulatory bodies.</li><li>
Enter relevant information/changes and renewable information into Solutions.</li><li>
Serve as liaison to TDFPS Residential Child Care Licensing for sampling visits and other related inspections for the foster/adoptive home.</li><li>
Ensure all documentation is entered into electronic health record. Monitor frequently to ensure facility and/or client compliance.</li><li>
Maintain current child record meets all recording requirements of the agency.</li><li>
Provide monitoring of foster home specific to children&rsquo;s plan of service and supervision needs as well as compliance with the physical home in accordance with Minimum Standards.</li><li>
Coordinate treatment by consulting and collaborating with referral sources, teachers, school counselors, and other professionals involved with the client.</li><li>
Meet productivity expectations by maintaining assigned caseload and managing schedule to meet appropriate timeliness.</li></ul><p><b>
Required Qualifications</b>:</p><ul><li>
Bachelor&rsquo;s degree, preferably in social work, or other human services field.</li><li>
Two (2) years of full-time documented experience in child welfare services conducting assessments, service planning, case management, and crisis management preferred.</li></ul><p><b>

What You'll Do</b></p>
<p>
As a Behavioral Health Care Manager (BHCM) with Imagine Pediatrics, you will work with the families of medically complex children providing case management services in accordance with Case Management Society of American (CMSA) Standards of Practice for members enrolled in Imagine Pediatrics behavioral health program. You will work alongside pediatricians, nurses, care coordinators, and other healthcare professionals. Your primary responsibilities will include:</p>
<ul>
<li>Monitor high-risk pediatric patients (up to 19 years old), some recently discharged from the hospital, ensuring appropriate follow-up and clinical management, and adjusting care plans as needed.</li>
<li>Conduct biopsychosocial assessments to address behavioral, social, emotional, and systemic needs of the patient and family.</li>
<li>Create and evaluate the effectiveness of the patient/family's care plan and modify based on families evolving needs and goal progression.</li>
<li>Provide intervention that is consistent with the social/emotional/physical needs of patients and caregivers such as mental health crises, behavioral issues, and family conflict.</li>
<li>Facilitate case management and support that requires clinical expertise in various systems with focus on helping patients and families negotiate the complexities involved with a mental health diagnosis.</li>
<li>Resource validated external services requested by the family to meet behavioral and social needs such as social services agencies and behavioral specialists.</li>
<li>Provides interventions in response to crisis to de-escalate and stabilize patient and family members</li>
<li>Provides psychoeducation on the nature of mental health diagnosis and progression, the importance of treatment adherence, and related information as appropriate</li>
<li>Collaborate with external care team members regularly including school systems, specialists, and DFPS as needed.</li>
<li>Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress</li>
<li>Represent Imagine Pediatrics commendably to patients, families, providers, and community</li>
<li>Performs other duties and assumes other responsibilities as assigned by manager</li>
