  Health Home Plus (HH+) is an intensive Health Home Care Management (HHCM) service established for defined populations with Serious Mental Illness (SMI) who are enrolled in a Health Home (HH) serving adults.

   What The Project Coordinator Does:

  The person filling this position is responsible for: aiding qualified individuals who require social service assistance. This includes interviewing and evaluating clients, formulating service plans and goals, in order to aid clients in meeting their Mental Health and Medical needs. This should be done through the use of appropriate community resources/referrals. Field work is mandatory, each client must be seen a minimum of 2 times per month or as needed, specific to client need.&nbsp; Project Coordinators are expected to provide intensive case management services to select populations&nbsp;

   (as defined by the New York State Department of Health)&nbsp;

  who also have a Serious Mental Illness (SMI) and are enrolled in a Health Home (HH) serving adults. Case Management services are provided for a period of up to 1 year, possibly longer, dependent on client need.

   Carry and maintain a caseload of up to 20 clients, providing assessments, developing care plans, provide service referral/navigation, and crisis intervention, as needed

   Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information
  </li>
  <li>
   Conduct required field work. All clients must be seen, in-person, a minimum of two times per month. Additional visits may be required depending on client need

   They would also require two additional telephonic calls, for a total of 4 encounters per month

   Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency

The Crane Safe Haven
<br><br>
Located in Manhattan, the Crane Safe Haven serves 100 individuals who are not yet ready to transition into permanent housing, as well as those who may have struggled in other housing programs. The Crane Safe Haven offers an alternative to life on the street: private, safe, clean, and affordable short-term accommodations. On-site case management and clinical services are available to meet the needs of its residents and to help those ready to move into secure permanent housing.

Reporting to the Clinical Coordinator, the Case Manager, Transitional Housing will provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.
<br><br>
This position has a 35-hour work week.
<br><br>
<b>ESSENTIAL DUTIES:</b><br>
<ul>
<li>Provide direct services to a caseload of approximately 15 clients, including conducting necessary assessments, benefits assistance, and other services necessary to support the client in achieving housing and recovery goals</li>
<li>Conduct psychosocial evaluations</li>
<li>Connect clients to psychiatric services</li>
<li>Connect clients to medical treatment and substance use treatment facilities when applicable</li>
<li>Obtain identification, public benefits, and other applicable income sources for clients</li>
<li>Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans</li>
<li>Prepare all documentation for housing applications</li>
<li>Assist and counsel clients in the process of obtaining permanent housing</li>
<li>Maintain record keeping in compliance with agency standards as well as those of City and State agencies</li>
<li>Intervene in crisis situations</li>
<li>Escort clients to appointments when needed</li>
<li>Upon training, provide assistance with self-administration of medication consistent with Breaking Ground's policies and procedures</li>

<b>MINIMUM QUALIFICATIONS:</b><br>
