PURPOSE: 
The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. Our work is guided by principles that underscore the critical importance of stable housing, use a culturally competent and anti-racist lens, and respect the dignity and legitimacy of each individual’s journey.
 
POSITION OVERVIEW: 
The Case Manager is the most senior level of direct care staff working within the Jewish Board’s supportive housing programs to help residents live in the community as independently as possible.
 
KEY ESSENTIAL FUNCTIONS: 
Resident Support
 <ul>
<li>Carries a caseload and practices person-centered and collaborative service planning with residents.</li>
<li>Is assigned the most complex clients; carries a larger caseload than case associates and case assistants</li>
<li>Ensures service plans are completed and signed by residents according to regulatory guidelines and standards.</li>
<li>Evaluates and assesses independent living skills using a detailed functional assessment tool or supports residents in using tool for self-assessment. Supports residents in meeting individual goals.</li>
<li>Supports residents toward the goal of medication management independence, including appropriate person-centered interventions.</li>
<li>Advocates on behalf of residents with other services providers and in a way that takes into account health disparities and systemic racism.</li>
<li>Interprets and explains agency policies and procedures to residents.</li>
<li>May run resident support groups.</li>
<li>Participates in monthly collaborative walk-throughs of unit/apartment with residents.</li>
<li>Escorts residents to appointments, as needed.</li>
<li>Facilitates monthly roommate meetings, when appropriate.</li>
<li>Assists residents with physically demanding tasks, such as carrying groceries, laundry, and other household items, as needed.</li>
<li>Heats fully cooked foods, on occasion.</li>
<li>Resolves crises and de-escalates situations threatening resident comfort and well-being.</li>
<li>Creates management plans to improve safety and prevent ongoing crises.</li>
<li>For congregate settings only: Performs safety rounds of the program and wellness checks, as necessary.</li>

Administrative Support and Coordination

<li>Provides services to those residents within the scope of program regulations and agency mission.</li>
<li>Independently completes regular and timely documentation of all resident interactions, including assessments, service plans and progress, and informational notes.</li>
<li>Ensures progress notes meet monthly billing documentation requirements, if necessary.</li>
<li>Files incidents reports, as appropriate.</li>
<li>Links residents to community-based treatment and collaborates with other services providers when coordinating services.</li>
<li>Participates in planning and execution of special and holiday events.</li>
<li>Participates in regular staff meetings and trainings.</li>
<li>Travels to off-site meetings and trainings, as needed.</li>
<li>Participates in intake, assessments, discharge, and follow-up conferences; may take the lead in presenting cases in these settings or in high-risk case meetings</li>

<div>The Case Manager provides a full range of direct services to clients living on the streets and pursuing permanent housing. The Case Manager will work within an interdisciplinary team focusing on moving people off the streets of Upper Manhattan into permanent housing and successfully reintegrating into their communities. Essential functions include, but are not limited to:

The Ali Forney Center (AFC) protects LGBTQ+ youth from homelessness and helps them build the tools needed to live independently. Founded in 2002, AFC provides housing, healthcare, and educational support to thousands of youth each year. AFC creates an affirming, supportive environment where LGBTQ+ youth can thrive.
POSITION SUMMARY
The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.
The primary focus areas include housing stability, crisis response, and service coordination.
WORK SCHEDULE: Monday-Friday 10am-6pm with one late night 12p-8p. Some weekends and holidays required
WORK FLEXIBILITY: 1 remote day per week 
KEY RESPONSIBILITIES<ul><li>
Responsible for recordkeeping and documentation, maintaining client charts, and compiling statistics for regular program reporting and data entry into contract databases.</li><li>

Ensure members of the DOSO staff are prepared with essential information and background for each meeting and/or trip. Draft travel itineraries, reserve lodging, set up meeting rooms, and handle all logistics associated with these activities. Prepare and process travel and expense reports in accordance with Institute policies</li>
<li>Record Management: Develop and maintain an efficient, confidential records management system to ensure immediate availability of materials. Assist with the maintenance and coordination of judicial and/or student intervention case files in case management system (Maxient)</li>
<li>Train, supervise and schedule work-study students. Assure that work is done in a timely manner and evaluate performance. Prepare all work-study paperwork coordinating with financial aid and payroll. Prepare student, bi-weekly and semi-monthly payroll</li>
<li>Utilize enterprise-wide Institute systems (Banner and Data Warehouse) and subsidiary systems to monitor financial activity and provide reports to the DOSO and the Student Experience Business Manager. Monitor spending to ensure adherence to general policies and guidelines. Identify and resolve discrepancies in accordance with Institute and DOSO portfolio standards and policies</li>
<li>Maintain inventory of office supplies which includes ordering and invoice processing through OSCAR and Banner</li>
<li>Coordinate communication, marketing, and social media efforts for any DOSO programming and educational initiatives. Arrange event details including but not limited to purchasing supplies or equipment, catering, reserving space, setting up and tearing down</li>
<li>Ensure that public spaces in Academy Hall are adequately maintained by inspecting public areas on a weekly basis and initiating service requests as needed. Monitor any possible safety concerns. Coordinate with the Student Experience Operations Manager if required to manage larger repair projects</li>
