PURPOSE: 
The Kaplan House (Manhattan) provides therapeutic and support services to adolescent’s ages 17-21 who are referred to us through ACS and the Juvenile Justice system. Our residents have experienced significant trauma, resulting in behavioral, emotional, and functional/ development issues. Kaplan House offers an extensive independent living skills program, including vocational preparation, and educational services that assists youth towards reaching independence.
 
POSITION OVERVIEW: 
The Case Planner is the most senior level of direct care staff working with clients of the Jewish Board’s children’s community residences residential programs. The Case Planner provides support and counseling with goals of stabilization, incorporation of independent living skills, and successful discharge to the community.
 
KEY ESSENTIAL FUNCTIONS:Client Engagement: 
<ul>
<li>Carries a caseload of residents and provides services to those residents within the scope of program regulations and agency mission.</li>
<li>Evaluates and assesses independent living skills and supports clients in meeting individual goals.</li>
<li>Advocates on behalf of residents with other services providers.</li>
<li>Links residents to community-based treatment and collaborate with other services providers when coordinating services.</li>
<li>Practices person-centered and collaborative service planning with clients, also ensuring service plans are completed and signed by residents according to regulatory guidelines and standards.</li>
<li>Interprets and explains agency policies and procedures to residents.</li>
<li>May run groups under supervision.</li>
<li>Supports residents toward the goal of medication management independence, including appropriate person-centered interventions.</li>
<li>Accompanies residents to appointments as needed.</li>

Team Process: 
<ul>
<li>Participates in intake, assessment, discharge, and follow-up conferences.</li>
<li>Participates in regular staff meetings and trainings. May be asked to travel to offsite meetings and trainings.</li>

DOCUMENTATION:

<li>Completes regular and timely documentation of all resident interactions, including progress and informational notes and assessments.</li>

Janian&rsquo;s primary care program, started over 10 years ago, continues to expand rapidly. Janian Primary Care Providers provide quality primary care in a variety of settings including permanent supportive housing, transitional shelters, safe havens, and drop-in centers, as well as within the community through Street Medicine. Primary Care Providers work closely with supportive staff as well as collaborate with behavioral health providers, social workers and case managers at program sites and residences, and street outreach teams who identify and refer medically vulnerable clients.</div>

MD, Board Certified or Board Eligible provider licensed to practice medicine in New York State, with training in family medicine, internal medicine, or other primary care specialty</li><li>

Heart To Heart Children's Services is a NYS approved Early Intervention agency. We provide services to children as little as the age of zero all the way to three years old.

We have a full team of staff who are here to assist our providers in ensuring they are able to successfully deliver services. We also have a team of clinical managers that will help support our providers! We offer around the clock access to our providers to make sure they have help and our successfully able to complete their sessions.
Provider Responsibilities
The Special Education Teacher will be working one on one with babies and toddlers who qualify for Early Intervention services within New York City.
Providers are expected to utilize their clinical knowledge and skills to help the child reach their milestones, as well as, to educate and guide parents/caregivers to ensure successful implementation between sessions.
Who qualifies as a Special Education Provider?<ul><li>
A Master's degree in Special Education from an accredited university.</li><li>
Valid Teaching Certification: Birth - Grade 2 Students with Disabilities certificate</li></ul>
