Required Qualifications</b></h2><ul><li><p>
Active, unrestricted <b>Nurse Practitioner license</b> in good standing.</p></li><li><p><b>
Family Nurse Practitioner (FNP) certification required.</b></p></li><li><p>
Current national board certification as an FNP.</p></li><li><p>
Minimum of <b>2 years of clinical Nurse Practitioner experience</b> in one or more of the following:</p><ul><li><p>

Emergency Medicine</p></li><li><p>
Acute Care</p></li><li><p>
Telehealth</p></li></ul></li><li><p>
Strong independent clinical judgment and the ability to make sound clinical decisions in a telehealth environment.</p></li><li><p>
Comfortable assessing and managing patients when traditional in-person physical examination findings are not available.</p></li><li><p>
Experience managing high-volume EHR inbox workflows and clinical communications.</p></li><li><p>
Excellent written and verbal communication skills.</p></li><li><p>
Strong organizational skills and ability to prioritize multiple clinical tasks effectively.</p></li><li><p>
Ability to work independently while collaborating effectively with a multidisciplinary care team.</p></li><li><p>
Reliable high-speed internet connection.</p></li><li><p>

Preferred Qualifications</b></h2><ul><li><p><b>
Emergency Nurse Practitioner Certification (ENP-C)</b> in addition to FNP certification.</p></li><li><p>
Active <b>Texas Nurse Practitioner license</b>.</p></li><li><p>
Multi-state Nurse Practitioner licensure.</p></li><li><p>
Experience with <b>Athena EHR</b> or similar electronic health record systems.</p></li><li><p>
Experience providing care in mobile medicine, home-based care, hospital-at-home, urgent care, emergency medicine, or transitional care settings.</p></li><li><p>
Experience with Transitional Care Management (TCM).</p></li><li><p>
Experience collaborating with field-based clinicians and multidisciplinary care teams.</p></li></ul><h2 class="jobSectionHeader"><b>
Licensure &amp; Multi-State Coverage</b></h2><p>
An active Texas NP license is preferred for this position. Multi-state licensure is strongly preferred and may expand the ability to provide telehealth services across CHP's growing service area.</p><p>
The selected candidate must be willing to obtain additional state licensure, as directed and supported by CHP, to meet program growth and patient coverage needs.</p><p>
Candidates are responsible for maintaining all required licenses, certifications, registrations, and credentials necessary to practice within each applicable jurisdiction.</p><h2 class="jobSectionHeader"><b>
