What You'll Do</div>

<div></div><div>The YWCA Community Jobs program works in partnership with DSHS to provide paid work experience to help people on TANF transition from state assistance to full-time employment. The Employment Specialist works with parents on TANF referred by DSHS to acquire workplace readiness skills, remove barriers to employment, obtain needed training and support services, and find unsubsidized employment that will enable self-sufficiency within a 6-9 month timeframe.</div>

<div>The Employment Specialist also works closely with non-profit, government and tribal worksites, and worksite supervisors to ensure a valuable subsidized work experience for each participant. In addition, the Employment Specialist maintains extensive records of clients&rsquo; progress and reports on achievement of program outcomes using the E-Jas operating system and must following strict timelines and meet report deadlines. The Case Manager for the Bills Bed/Unified Care Team program will provide trauma-informed care, case management, and general and intensive social services for persons self-identifying as male&mdash;often with co-occurring disorders&mdash;who are experiencing homelessness, which includes all or part of a comprehensive housing readiness assessment (HRA), individualized service plans (ISP), and service-intensive case management intended to assist the individual towards self-sufficiency. The UCT program engages clients using strength-based, client-centered methodologies to establish an alliance to assure that the client is respected, motivated, and supported to reach goals identified by the client and the case manager.

DUTIES &amp; RESPONSIBILITIES:

<li>Provide comprehensive case management that includes screening and assessing clients; creating, implementing, and monitoring individual service plans; crisis intervention; and connecting clients to health, mental health, employment, housing, and other community resources.</li>
<li>Provide case management for clients with varying degrees of vocational, social, cultural, and/or medical impairments hindering economic independence in a goal-directed, task-centered, and time-limited manner.</li>
<li>Provide information regarding services and provide transportation as needed to people seeking services.</li>
<li>Continually monitor and evaluate each client&rsquo;s progression through their case management plan and develop corrected action revisions to the plan as needed.</li>
<li>Asist clients with applications for Social Security or any state assistance.</li>
<li>Foster a professional and welcoming environment in all interactions with clients, guests, vendors, and colleagues, regardless of differing identities, backgrounds, beliefs, and choices.</li>
<li>Participate in training as assigned and coordinate in-house training for staff.</li>

<li>Serve as a mentor to fellow staff members, including the Case Manager&rsquo;s Assistants (CMA).</li>
<li>Consult on client cases as needed, provide support for complex cases, and report pertinent information to the Program Manager.</li>

<li>Assist the Program Manager with the development of case management standards, guidelines, and paperwork to assure high-quality care management to program participants.</li>
<li>Assist with developing an after-care Housing Stability Plan and identifying the steps required to accomplish goals to move the person from homelessness to self-sufficiency. Successfully exit clients from the program using progressive engagement strategies.</li>
<li>Maintain weekly contact with caseload for the first thirty dates of program participation.</li>
<li>Keep accurate and timely case management and staffing notes, as well as organized and confidential records. Input client case records in WellSky, HMIS, or other database systems. Ensure that case records, social services systems (WellSky and HMIS) and other required forms and reports are maintained in the prescribed format.</li>
<li>Complete the following training: The Salvation Army driving training, The Salvation Army&rsquo;s Protecting the Mission training, HIPPA Compliance training, Clarity (HMIS) and WellSky database training.</li>

EDUCATION &amp; EXPERIENCE:
<ul>
<li>Bachelor&rsquo;s degree in social work or human services. Three plus years of experience in case management, housing navigation, or similar experience will also be considered.</li>
<li>Experience in three or more of the following fields: human services, veterans&rsquo; services, case management, housing navigation, community building, and knowledge of resources available for persons experiencing homelessness that may have co-morbidities.</li>

JOB DEFINITION:
Support, Advocacy, Growth and Employment (SAGE) is DESC's comprehensive outpatient mental health program. SAGE intake specialists, case managers, psychiatrists and nurses assist adults with severe and persistent mental illness in their journey towards recovery. Long-term case management services are provided including help with obtaining and maintaining housing, improving clinical and social stability, and enriching their level of independence and self-sufficiency.
SAGE services are licensed under the Washington State Division of Behavioral Health and Recovery and are provided under contract with King County Mental Health, Chemical Abuse &amp; Dependency Services Division. Services are provided within a Recovery model.
MAJOR DUTIES AND RESPONSIBILITIES:
Case Management:<ul><li>
Participate as a member of a multi-disciplinary and multi-departmental team providing on-going case management services to adults with severe and persistent mental illnesses.</li><li>
Collaborate with DESC&rsquo;s Scattered Sites Supportive Housing team in the development of client care and housing retention plans.</li><li>
Provide outreach and engagement services to assigned clients, to initially engage clients and maintain working relationships.</li><li>
Conduct psychosocial assessments; develop goal planners with consumer participation; facilitate linkages to collaborative resources when appropriate.</li><li>
Assume primary responsibility for coordinating all aspects of consumers' support and goal planners. This includes: providing and/or coordinating the delivery of psychiatric treatment; substance abuse treatment; 24-hour support services; and performing acute case management to facilitate consumer access to food, clothing, housing, medical care, financial assistance and vocational training or employment.</li><li>

MINIMUM QUALIFICATIONS:<ul><li>

Key Responsibilities:
