The Ali Forney Center (AFC) protects LGBTQ+ youth from homelessness and helps them build the tools needed to live independently. POSITION SUMMARY
The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.
The primary focus areas include housing stability, crisis response, and service coordination.
WORK SCHEDULE: Monday-Friday 10am-6pm with one late night 12p-8p. Some weekends and holidays required
WORK FLEXIBILITY: 1 remote day per week 
KEY RESPONSIBILITIES<ul><li>
Responsible for recordkeeping and documentation, maintaining client charts, and compiling statistics for regular program reporting and data entry into contract databases.</li><li>
Facilitate comprehensive assessments of residents&rsquo; needs, strengths, and goals.</li><li>
Develop and implement Individualized Service Plans that address housing, mental health, vital documents, legal, entitlement benefits, education, employment, and health needs.</li><li>
Provide crisis intervention and ongoing support.</li><li>
Collaborate with Ali Forney Center&rsquo;s Residential Treatment Team, Health Services,Mental Health, and other relevant teams to coordinate comprehensive care for residents.</li><li>

Assist residents in accessing housing resources, employment opportunities, educational programs, and health care services by accompaniment as needed.</li><li>
Participate in regular supervision, training, and professional development activities.</li><li>
Attend and participate in weekly treatment planning meetings, housing staff meetings, supervision, and other relevant meetings for this role.</li><li>
Develop relationships with NYC service providers, including but not limited to those addressing social, educational/vocational, medical, mental health, and housing needs.</li><li>

QUALIFICATIONS<ul><li>
Must have a High School diploma or equivalency required.</li><li>
Computer knowledge and competent writing skills.</li><li>
6 months-1 year experience in residential housing, supportive housing, shelter, transitional living, or youth development programs.</li><li>

<div>The Case Manager provides a full range of direct services to clients living on the streets and pursuing permanent housing. The Case Manager will work within an interdisciplinary team focusing on moving people off the streets of Upper Manhattan into permanent housing and successfully reintegrating into their communities. Essential functions include, but are not limited to:

The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. Our work is guided by principles that underscore the critical importance of stable housing, use a culturally competent and anti-racist lens, and respect the dignity and legitimacy of each individual’s journey.
 
POSITION OVERVIEW: 
The Case Manager is the most senior level of direct care staff working within the Jewish Board’s supportive housing programs to help residents live in the community as independently as possible.
 
KEY ESSENTIAL FUNCTIONS: 
Resident Support
 <ul>
<li>Carries a caseload and practices person-centered and collaborative service planning with residents.</li>
<li>Is assigned the most complex clients; carries a larger caseload than case associates and case assistants</li>

About the Position
 
The Case Manager will be responsible for providing residential and long term support services to homeless and at-risk adults transitioning into permanent housing. The Case Manager will carry a caseload of approximately 20 to 25 formerly homeless and/or low income individuals. 
Case Manager will be required to have frequent contact with tenants, weekly and/or monthly face-to-face meetings, telephone conversations, and aftercare contacts. Frequent electronic exchanges with colleagues is expected .
 
 
Responsibilities:
 
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li><li>Using a person centered approach, provide case management services and assessments rooted in evidence based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.</li><li>Utilize electronic health record. MINIMUM QUALIFICATIONS: 
<ul>
<li>NYS Licensed Masters Social Worker (LMSW) required</li>
<li>Minimum 3 years of experience with related populations; previous supervisory and administrative experience preferred</li>
<li>Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients</li>
<li>Experience with crisis intervention, including, risk assessments and incident management</li>
<li>Ability to work successfully with a wide range of internal and external stakeholders</li>
<li>Demonstrated success in working in a fast-paced environment with multiple priorities</li>
<li>Ability to delegate and motivate staff to achieve deliverables</li>
<li>Ability to communicate (verbally and written) with diverse populations and stakeholders</li>
<li>Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; familiarity with Access-based databases and the ability to learn new programs are required</li>

(WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops, manages, and provides housing for low‐ income older persons, many of whom live with mental illness and/or have experienced homelessness.
Position Description
The Staff Engagement and Culture Manager plays a key role in cultivating a connected, inclusive, and engaging employee experience by leading programs and initiatives that strengthen culture, recognition, and community at WSFSSH.
Reporting to the Managing Director of DEI &amp; Organizational Learning &amp; Development, this individual contributor leads the planning, implementation, and continuous improvement of employee engagement and culture-building initiatives. Working collaboratively across departments and with leaders across multiple sites, the role combines project management, relationship building, and operational excellence to deliver exceptional employee experiences and strengthen organizational culture.

Job Duties &amp; Responsibilities
Employee Engagement &amp; Culture<ul><li>
Lead the implementation and ongoing management of WSFSSH's signature employee engagement programs and culture-building initiatives.</li><li>
Partner with the Managing Director to develop annual engagement strategies, work plans, event calendars, and implementation timelines aligned with organizational priorities.</li><li>
Identify opportunities to enhance and expand employee engagement programs, recognition strategies, and culture-building initiatives.</li><li>
Champion a culture of recognition, inclusion, collaboration, and belonging through programs that foster meaningful employee experiences across the organization.</li><li>
Coordinate and support employee affinity groups, employee-led initiatives, and other community-building programs that advance the organization's Diversity, Equity, and Inclusion goals byprovidingoperational guidance, resources, and logistical support.</li><li>
Oversee all programlogistics, including scheduling, cross-site coordination, vendor management, communications, registrations, catering, and day-of execution.</li></ul>
Employee Recognition, Events &amp; Community<ul><li>
Partner with the Managing Director to coordinate year-round employee recognition and appreciation initiatives, including recognition campaigns, milestone celebrations, employee appreciation events, cultural celebrations, and the procurement and distribution of awards, gifts, and promotional materials.</li><li>
Lead the planning and execution of WSFSSH's Annual Fall Fest and support other special organization-wide celebrations.</li><li>
Manage the full event lifecycle, including budgeting, venue selection, vendor coordination, branded merchandise, catering, entertainment, communications, registration,logistics, and on-site execution.</li><li>
Ensure staff events create welcoming and inclusive experiences that reflect WSFSSH&rsquo;s mission, values, and culture.</li></ul>
Partnerships, Employee Experience, &amp; Program Management<ul><li>
Serve as a trusted partner to the Managing Director and other leaders across the organization, helping departmentsidentifyopportunities to strengthen employee connection, recognition, and engagement.</li><li>
Build andmaintaincollaborative relationships with internal stakeholders, community partners, and vendors.</li><li>
Partner with the Communications Manager to develop internal communication strategies that increase awareness, participation, and excitement around engagement initiatives and organizational events.</li><li>
Gather employee feedback and track participation and engagement metrics to measure program effectiveness,identifytrends, and inform continuous improvement.</li><li>
Manage program and event budgets, procurement, contracts, vendor relationships, and administrative operationsin accordance withorganizational policies.</li><li>
Maintain program documentation, implementation timelines, and operational processes that ensure the successful delivery of engagement programs and events.</li><li>
Support new organization initiatives, culture-building efforts, and special programs that advance WSFSSH&rsquo;s strategic plan and organizational priorities.</li></ul>
