<div>The Case Manager provides a full range of direct services to clients living on the streets and pursuing permanent housing. The Case Manager will work within an interdisciplinary team focusing on moving people off the streets of Upper Manhattan into permanent housing and successfully reintegrating into their communities. The Case Manager&rsquo;s responsibilities will include outreach to clients living on the street as well assisting their caseload with obtaining permanent housing, helping clients live full and satisfying lives in the community, helping their team and program to function well and meeting their contractual obligations including achieving contracted housing placement goals, and striving as appropriate to their role, to fulfill the CUCS programs&rsquo; core purpose, core values and vision statements.</div><br>

Essential functions include, but are not limited to:</b></p>

Develop and maintain collaborative, professional relationships with clients while creating, implementing, and regularly updating individualized service plans that reflect each client's strengths, goals, and needs.</li><li>
Conduct ongoing assessments and monitor client progress, adjusting service plans as appropriate to support long-term stability and recovery.</li><li>
Coordinate comprehensive services and referrals to address housing, healthcare, behavioral health, substance use, employment, education, financial, and social service needs.</li><li>
Assist clients with obtaining and maintaining public benefits by completing applications, coordinating appointments, monitoring renewals, and advocating with federal, state, and local agencies.</li><li>
Support clients in securing and maintaining permanent housing, including housing searches, application completion, move-in coordination, landlord communication, annual recertifications, apartment inspections, and tenancy preservation.</li><li>
Provide crisis intervention, institutional visits, discharge planning, and coordination with hospitals, shelters, emergency responders, and other community providers.</li><li>
Deliver services both onsite and throughout the community, including home visits and accompanying clients to medical appointments, housing interviews, court appearances, benefits offices, schools, job interviews, and other essential appointments.</li><li>
Facilitate individual and group services, including recreational activities, psychoeducational groups, evidence-based interventions, and wellness programming.</li><li>
Support clients with activities of daily living, including personal care, housekeeping, laundry, hoarding mitigation, pest remediation, donation coordination, and maintaining safe living environments.</li><li>
Support new client admissions by assisting with orientation, distributing essential household items, processing belongings through the PackTite system, and assisting with bed bug remediation when necessary.</li><li>
Maintain timely, accurate, and comprehensive documentation, including progress notes, assessments, Individualized Living Plans (ILPs), benefit documentation, and other required records in accordance with agency, regulatory, and funder requirements.</li><li>
Maintain required monthly client contact standards and ensure all services are delivered in compliance with agency policies, licensing requirements, and contractual obligations.</li><li>
Protect client confidentiality and uphold all ethical, professional, and regulatory standards.</li><li>
Participate in required trainings, team meetings, supervision, and professional development activities.</li><li>

Reporting to the Assistant Program Director, Transitional Housing the Clinical Coordinator, Transitional Housing supervises Case Managers and ensures the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. This is a fabulous opportunity for a recent MSW graduate interested in working with children in a therapeutic setting. You’ll be responsible for completing psychosocial assessments and providing mental health counseling and crisis intervention services to children and their families.

The Division of Building and Land Development Services (BLDS) leads the agency&rsquo;s effort in providing architectural, engineering, cost valuation, environmental planning, and construction support services to the various divisions within HPD&rsquo;s Office of Development. The Office of Development utilizes a public-private partnership model and provides loans, grants, and/or incentives to assist in the finance of housing development projects that will benefit low- and moderate-income New Yorkers.

BLDS is the largest division within the Office of Development with over 100 staff, and is composed of seven units, including: the Bureau of New Construction Design Services, the Bureau of Preservation Design Services, the Bureau of Engineering, the Bureau of Construction Services, the Environmental Planning Unit, the Program Management Unit, and the Executive Office.

As Data Analyst, you will report to the Deputy Director of Analytics, within the BLDS Executive Office. You will work with the six other BLDS units to facilitate the analysis of internal and cross-agency data, while identifying trends in housing development projects. In addition, you will develop metrics and prepare reports and analyses related to performance evaluation, compliance, projects, or other initiatives.

Your Responsibilities:

<ul><li>Data management: maintaining regular reporting and analyses, including bi/weekly MMR reports, project summary analyses, and annual reports.
</li><li>Performance management &amp; evaluation: developing metrics on internal (BLDS) and external (Architects, Contractors, etc.) performance and workloads, resulting in new or updated analyses and summary reports, among others.
</li><li>Creating data visualization tools and materials for presentations.
</li><li>Maintenance of BLDS&rsquo; data systems, including monitoring reports, updating user guides and other informational documents, providing technical/procedural guidance, and troubleshooting, as needed.

The OOD is comprised of eight divisions that administer a range of programs. Under the supervision of the Executive Director of the Bureau of Engineering, the Deputy Director will be responsible for reviewing, and overseeing the MEP unit responsible for reviewing, the design and monitoring the installation of mechanical systems for both newly-constructed and rehabilitated multi-family buildings. This role also involves coordination with internal and external stakeholders to address complex conditions in existing buildings and support effective project delivery.

The ideal candidates should have a background in Construction Management with a specialization in the installation of mechanical systems including elevator systems and possess a thorough understanding of design and construction means and methods for multi-family housing in New York City

Your Responsibilities:
<br>
<br>
<ul><li>Supervising the MEP Unit and undertaking the review of mechanical, electrical, plumbing, and sprinkler plans and contract documents, submitted by private architects and engineers, to assure compliance with industry design standards and pertinent codes, including the New York City Building Code, ASHRAE, New York City Energy Conservation Code, and Enterprise Green Communities.
</li><li>Assure that industry and energy-efficient standards are being met or exceeded in the design of mechanical, electrical, plumbing, and sprinkler systems.
</li><li>Attend design consultations with internal and external stakeholders and provide technical MEP feedback on proposed design and scope of work.
</li><li>Conduct field surveys and inspections of existing mechanical, electrical, plumbing, and sprinkler systems, assist in determining the scope of work, cost estimates, and provide recommendations for repair and/or systems replacement.
</li><li>Monitor the installation of mechanical, electrical, plumbing, and sprinkler systems during the construction/alteration of multi-family buildings to ensure conformance of work to approved plans, contract specifications, industry standards and pertinent codes, and timely adherence to the proposed construction schedule.
</li><li>Review scopes of work for mechanical systems for errors and omissions.
</li><li>Perform thorough and systematic research on mechanical systems by obtaining, reviewing and analyzing pertinent information, documentation and consulting appropriate parties to successfully resolve discrepancies.
</li><li>Prepare contract documents for mechanical, electrical, plumbing, and sprinkler systems in multiple dwelling and city-owned buildings.
</li><li>Prepare accurate, organized and comprehensive reports pertaining to engineering matters.
</li><li>Engage in research, investigations, studies and analysis related to engineering functions of the Agency.
</li><li>Coordinate with consultants, developers, and internal program staff to resolve MEP-related issues in rehabilitation and preservation projects involving existing building systems.
