This class of positions encompasses professional, supervisory or administrative work of varying degrees of difficulty and responsibility in the operation of a social work department providing psychosocial and other social work services to different types of patients across various settings. Under general direction, with very wide latitude for independent judgment and decision-making, performs the following:
Examples of Typical Tasks:
Performs and supervises the duties of Assignment Level I:
1. Screens charts for historical and clinical information, conducts psychosocial evaluations through interviews with patients and/or identified surrogates and formulates appropriate social work treatment plans.
2. Works within health care setting offices or in community settings within NYC to provide individual and group counseling, as well as concrete social services, as required.

3. Maintains individualized supervision logs documenting clinical supervision and ensures timely completion of attestation forms required by NYSED Office of the Professions to support LCSW licensure applications, as applicable.

5. Establishes and maintains professional social work standards of performance concerning interview techniques and methods and practices used in preparing clinical assessments, developing treatment plans, and in documenting case histories

We're currently looking for an independently licensed mental health professional to join the Care Navigation team (LCSW, LMFT, LPC required) to support adult and adolescent members seeking support with posttraumatic stress disorder (PTSD) and complex trauma. Reporting to the clinical lead of the Trauma Specialty Care Program, the Trauma Specialty Care Navigator will provide initial assessment, support, referral, and ongoing care coordination for members within Spring Health's innovative Trauma program. The role may also support teens 13 to 17 and their caregivers.

This is a full time position that is fully remote. The primary shift for this position is Monday-Friday, 9-5pm PT.

What you'll do:

<li>Developmentally appropriate telephonic and telehealth screening, brief intervention, and referral of members to a trauma specialty treatment provider.</li>

What you'll bring:
<ul>
<li>Independent clinical mental health license firmly required (LCSW, LMFT, LPC, or equivalent).</li>
<li>Minimum of 2+ years of clinical care experience working with PTSD and Complex Trauma in adult and adolescent populations in outpatient or intensive outpatient settings.</li>
<li>Experience with motivational interviewing, diagnostic assessment, and triage.</li>
<li>Knowledge of phased-based trauma treatment models and EBPs for PTSD (e.g. Prolonged Exposure, Cognitive Processing Therapy, EMDR, ART, etc).</li>
<li>Experience working with parents and caregivers, including providing psychoeducation, motivational work, and collaborating with families to support the adolescent's treatment and overall wellbeing when the teen is the identified patient.</li>
<li>Excellent listening, written and verbal communication skills.</li>
<li>Acknowledge and embrace diversity and inclusion.</li>
<li>Experience working with diverse populations.</li>
<li>Motivated to go above and beyond for members to provide a high-touch experience.</li>
<li>Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks.</li>
<li>Able to multi-task and context switch!</li>

The target base salary range for this position is $33.33 - $42.31 Hourly, and is part of a competitive total rewards package including stock options and benefits. Individual pay may vary from the target range and is determined by a number of factors including experience, location, internal pay equity, and other relevant business considerations. We review all employee pay and compensation programs annually using Radford Global Compensation Database at minimum to ensure competitive and fair pay.
