We are seeking an experienced Telehealth Nurse Practitioner (NP) to join our Mobile Acute Care Program. This fully remote role provides direct virtual patient care while also supporting our mobile field clinicians through timely EHR inbox management, clinical coordination, and transitions of care.

Position Summary</h2>
The Telehealth Nurse Practitioner will provide virtual evaluation and management of patients with acute, urgent, and follow-up care needs. The NP will conduct telehealth visits, perform virtual assessments, develop diagnostic and treatment plans, prescribe medications within the scope of practice, and provide transitional care management following hospital or facility discharge.
In addition to direct patient care, this position will play an important role in supporting the Mobile Acute Care team by managing the EHR inbox, reviewing clinical messages and diagnostic results, responding to refill requests, and coordinating patient escalations and care plan updates with field clinicians.
This position will initially operate Monday through Friday during standard business hours, with weekend coverage anticipated as the program expands.<h2 class="jobSectionHeader">
Key Responsibilities</h2><ul><li>
Conduct telehealth visits with patients using a secure, HIPAA-compliant video and audio platform, including acute, urgent, and follow-up visits.</li><li>
Obtain comprehensive virtual patient histories and perform appropriate virtual assessments.</li><li>
Evaluate symptoms, establish differential diagnoses, and develop appropriate diagnostic and treatment plans.</li><li>
Conduct Transitional Care Management (TCM) visits for patients following discharge from hospitals, skilled nursing facilities, rehabilitation facilities, and other care settings.</li><li>
Order, review, and interpret laboratory testing, imaging, and other diagnostic studies as clinically indicated.</li><li>
