About the Position
 
The Case Manager will be responsible for providing residential and long term support services to homeless and at-risk adults transitioning into permanent housing. The Case Manager will carry a caseload of approximately 20 to 25 formerly homeless and/or low income individuals. 
Case Manager will be required to have frequent contact with tenants, weekly and/or monthly face-to-face meetings, telephone conversations, and aftercare contacts. Frequent electronic exchanges with colleagues is expected .
 
 
Responsibilities:
 
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li><li>Using a person centered approach, provide case management services and assessments rooted in evidence based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.</li><li>Utilize electronic health record. Maintain accurate and up-to-date tenant files/charts.</li><li>Coordinate and document clinical case management and psychosocial services while also documenting the overall effectiveness of the case management services provided. Case Managers will develop an Individual Service Plan and review the plan periodically to document process toward goals.</li><li>Serve as a liaison between tenant and community providers.</li><li>Facilitate groups with a focus on life skills, education and support, relapse prevention and mental health counseling.</li><li>Complete referrals to medical, mental health, and other outside providers.</li><li>Collaborate with Workforce Opportunities Staff to assist tenant in developing employment related goals, where applicable.</li><li>Ensure overall compliance with organizational and funder regulations. Adhere to documentation requirements as assigned by leadership and funder regulation/standard.</li><li>Coordinate aftercare services and follow-up contacts for post-discharge updates.</li><li>Conduct outreach and engagement including apartment visits and inspections.</li><li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li><li>Work with Property Management staff to assist tenant to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors. Mediate landlord-tenant disputes that threaten housing stability.</li><li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist tenants with overcoming barriers to housing stability.</li><li>Participate in community initiatives to fulfill mission to end homelessness.</li><li>Provide crisis intervention as needed.</li><li>Provide all services in a manner that respects cultural differences amongst tenants.</li><li>Carry out other duties as assigned by leadership.

Requirements:
 
<ul><li>A Bachelor's degree in human services or related field.</li><li>Must possess strong organizational, writing, communication, and computer skills.</li><li>Some relevant experience working with low-income or homeless populations.</li><li>Fluent in Spanish is required.</li><li>Candidates must be self-starters with the skills and energy to work within the community.</li><li>Case Manager is regularly required to, type, use hands and fingers, reach with hands and arm. The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. Our work is guided by principles that underscore the critical importance of stable housing, use a culturally competent and anti-racist lens, and respect the dignity and legitimacy of each individual’s journey.
 
POSITION OVERVIEW: 
The Case Manager is the most senior level of direct care staff working within the Jewish Board’s supportive housing programs to help residents live in the community as independently as possible.
 
KEY ESSENTIAL FUNCTIONS: 
Resident Support
 <ul>
<li>Carries a caseload and practices person-centered and collaborative service planning with residents.</li>
<li>Is assigned the most complex clients; carries a larger caseload than case associates and case assistants</li>
<li>Ensures service plans are completed and signed by residents according to regulatory guidelines and standards.</li>
<li>Evaluates and assesses independent living skills using a detailed functional assessment tool or supports residents in using tool for self-assessment. Supports residents in meeting individual goals.</li>
<li>Supports residents toward the goal of medication management independence, including appropriate person-centered interventions.</li>
<li>Advocates on behalf of residents with other services providers and in a way that takes into account health disparities and systemic racism.</li>
<li>Interprets and explains agency policies and procedures to residents.</li>
<li>May run resident support groups.</li>
<li>Participates in monthly collaborative walk-throughs of unit/apartment with residents.</li>
<li>Escorts residents to appointments, as needed.</li>
<li>Facilitates monthly roommate meetings, when appropriate.</li>
<li>Assists residents with physically demanding tasks, such as carrying groceries, laundry, and other household items, as needed.</li>
<li>Heats fully cooked foods, on occasion.</li>
<li>Resolves crises and de-escalates situations threatening resident comfort and well-being.</li>
<li>Creates management plans to improve safety and prevent ongoing crises.</li>
<li>For congregate settings only: Performs safety rounds of the program and wellness checks, as necessary.</li>

Administrative Support and Coordination

<li>Provides services to those residents within the scope of program regulations and agency mission.</li>
<li>Independently completes regular and timely documentation of all resident interactions, including assessments, service plans and progress, and informational notes.</li>
<li>Ensures progress notes meet monthly billing documentation requirements, if necessary.</li>
<li>Files incidents reports, as appropriate.</li>
<li>Links residents to community-based treatment and collaborates with other services providers when coordinating services.</li>

This position is responsible for supporting the Government&rsquo;s professionals in all aspects of litigation support and e-discovery processes, including providing project management, Electronically Stored Information (ESI) intake, data analysis, early case assessment, document production, and liaising with third-party vendors. Skills and attributes for success: - Performs administrative functions associated with the day-to-day use of litigation support software eDiscovery databases, including but not limited to: - database creation and maintenance; - importing images, data and transcripts; - searching and exporting data; - document scanning, OCR and coding; - and document productions - Processing and publishing of electronically stored information for cases. - Assists in the preparation of documents and exhibits for trial. - Receives and logs incoming media; maintains chain of custody and other tracking documentation for media and data, both received and produced. - Contributes to internal process development, preparing workflows and other documentation. - Ability to work overtime hours, including on nights and/or weekends, on a per-project/case basis. - Problem-solving skills. - Work directly with the General Counsel, Attorneys, Law Clerks, and Legal Assistants as part of the matter team. - Assist with troubleshooting of technical issues within the eDiscovery platform. - Maintain a working knowledge of the discovery industries&rsquo; best practices and new regulations through training, certification completion and networking. - Meet with and build relationships with eDiscovery vendors and maintain relationships with current vendors. - Desire to be self-motivated and eager to shape the future of the department. - Ability to learn new eDiscovery review platforms quickly. Qualifications: - An understanding of the litigation lifecycle and electronic discovery/document review processes, procedures and practices is required. - 7+ years of experience in a litigation support, electronic discovery and/or technology support environment, preferably within the Government, but a law firm or top eDiscovery vendor is also acceptable. - Experience performing Administrator and Case Manager functions in Relativity. - Experience with providing end-user support on all Litigation Support applications, including but not limited to assisting with end-user training. - Experience with Relativity, Nuix, Concordance, IPRO and other applicable eDiscovery software platforms. - Must be a US Citizen. - Must be able to obtain a favorably adjudicated Public Trust Clearance.  The Administrative Coordinator manages and oversees the day-to-day operations and administrative support activities of the Dean of Students Office (DOSO), Conference Room, and Off-Campus Community Center. The Administrative Coordinator serves as the primary point of contact for both internal and external constituencies, assists and represents DOSO in communicating with constituents, and handles multiple issues and concerns as they arise. The Administrative Coordinator coordinates logistics for programming and completes special projects as needed by offices in the Dean of Students portfolio.

<ul><li>Manage daily office routines to ensure efficient and effective office operations. Triage incoming calls and serve as first point of contact. Provide DOSO staff with all relevant information to provide timely responses and follow-up. Assist the DOSO in planning, organizing prioritizing and coordinating the timely assignment, follow-up and completion of work and projects. Track and record leave time for DOSO. Ensure that new employees are appropriately processed for payroll and proper procedures are followed for employment separations</li>
<li>Plan and maintain complex, busy and shifting schedules of appointments, meetings and travel. Ensure members of the DOSO staff are prepared with essential information and background for each meeting and/or trip. Draft travel itineraries, reserve lodging, set up meeting rooms, and handle all logistics associated with these activities. Prepare and process travel and expense reports in accordance with Institute policies</li>
<li>Record Management: Develop and maintain an efficient, confidential records management system to ensure immediate availability of materials. Assist with the maintenance and coordination of judicial and/or student intervention case files in case management system (Maxient)</li>
<li>Train, supervise and schedule work-study students. Assure that work is done in a timely manner and evaluate performance. Prepare all work-study paperwork coordinating with financial aid and payroll. Prepare student, bi-weekly and semi-monthly payroll</li>
<li>Utilize enterprise-wide Institute systems (Banner and Data Warehouse) and subsidiary systems to monitor financial activity and provide reports to the DOSO and the Student Experience Business Manager. Monitor spending to ensure adherence to general policies and guidelines. Identify and resolve discrepancies in accordance with Institute and DOSO portfolio standards and policies</li>
<li>Maintain inventory of office supplies which includes ordering and invoice processing through OSCAR and Banner</li>
<li>Coordinate communication, marketing, and social media efforts for any DOSO programming and educational initiatives. Arrange event details including but not limited to purchasing supplies or equipment, catering, reserving space, setting up and tearing down</li>
<li>Ensure that public spaces in Academy Hall are adequately maintained by inspecting public areas on a weekly basis and initiating service requests as needed. Monitor any possible safety concerns. Coordinate with the Student Experience Operations Manager if required to manage larger repair projects</li>
<li>Other duties and special projects as assigned</li>
</ul>Shift:
Business Hours, Monday through Friday. Weekends or evenings occasionally required
Starting Salary/Rate:
Expected hiring range: $20.00 - $23.75 / hr
